摘  要:目的回顾性分析影响大面积脑梗死患者预后的预测因子。方法随访2007年1月至2008年12月确诊大面积脑梗死患者40例,分析影响梗死后生存时间的可能因素。结果 Kaplan-Meier分析示年龄≥71岁平均生存时间(16.62±2.98)个月,<71岁平均生存时间(30.60±1.59)个月;IMT≥0.80 mm平均生存时间(22.70±2.12)个月,IMT<0.80 mm平均生存时间(32.83±2.03)个月,差异均有统计学意义(P<0.05)。吸烟史、糖尿病、高血压及脂质代谢异常均影响梗死后生存时间(P<0.05)。Cox风险模型分析仅年龄和IMT可成为大面积梗死后生存独立预测因子(HR=1.185、16.461,P<0.05)。结论年龄和颈总动脉IMT可作为大面积脑梗死患者预后预测因子,且颈总动脉IMT预测风险优于年龄。Objective To retrospectively evaluate the prognostic factors that can influence long-term survival of patients who suffered large cerebral infarction.Methods Between January 2007 and December 2008,a total of 40 patients were diagnosed with a large cerebral infarction in this hospital.The multiple factors influencing the life expectancies of these 40 patients were investigated.Results In Kaplan-Meier analysis,the mean survival time of the older patients(≥71years) was 16.62±2.98 months,whereas that of the younger patients(71 years) was much better as 30.60±1.59 months(P=0.000).The mean survival time of the carotid IMT≥0.80 mm was 22.70±2.12 months,whereas that of the IMT0.80mm was 32.83±2.03 months(P=0.008).Involvement of smoking,diabetes,hypertension and dyslipidemia also resulted in a poor outcome(P0.05).However,other factors such as gender,infarction territory did not significantly influence the mean survival time in the current study(P0.05).Under Cox proportional hazard model analysis only age and carotid IMT had significantly influence the mean survival time after large cerebral infarction(P0.05).Conclusion Age and carotid IMT can be critical prognostic factors for survival after acute large cerebral infarction.But carotid IMT is superiority to age in prognosis.
