摘 要:目的:探讨两种基因工程乙肝疫苗加强接种的免疫效果。方法:选择244名52~60月龄出生时已全程接种乙肝疫苗的儿童,检测HBsAg、抗-HBc、ALT及抗-HBs滴度,随机分为A、B两组,分别加强接种一针重组酵母基因乙肝疫苗、细胞产基因工程乙肝疫苗,均为5μg,一月后复查抗-HBs滴度。结果:A、B两组加强后执-HBs阳转率分别为96.77%、100%,GMT较加强前分别上升10.71、12.26倍,表明两种基因乙肝疫苗加强免疫效果良好。A、B两组加强前抗-HBs已阴转的儿童加强免疫应答水平显著低于未阴转者(P<0.005)。B组同一等级抗-HBs滴度的儿童加强免疫应答显著优于A组。结论:国产两种基因工程乙肝疫苗于儿童52~60月龄时加强免疫效果可靠,是否表明细胞产乙肝基因疫苗加强免疫效果优于重组酵母乙肝基因疫苗,有待进一步观察。Objective: In order to study the immunoenhancement efficacy of two recombinant hepatitis B vaccines.Methods: Afer their HBsAg,Anti-HBc,ALT and Anti-HBs levels were examined. 244 children aged 52~60 months, inoculated with hepatitis B vaccine at birth, were assigned randomly either to group A or to group B. Children in group A and in group B were vaccinated with 5μg of yeas recombinant HB vaccine or cell recombinant HB vaccine respecitvely. Their Anti - HBs level was remeasurdd in a menth.Results: In group A and in group B, the positive rate of Anti - HBs was 96. 77% and 100% ,the level of Anti -HBs GMT was 10. 71 times and 12. 26 times as high as it was before reinoculation.But the level of Anti -HBs GMT of antibody - negative children before reinoculation was significantly lower than that of children who were antibody - positive before reinoculation in tow groups.In addition,the level of anti - HBs of some children in group A was higher than that of some children in group B. Conclusions: The immunoenhancement efficacy of two recombinant hepatitis B vaccines is satisfactory. However,whether the efficacy of cell recombinant HB vaccine is superior to that of yeast recombinant HB vaccine should be studied farther.
