胃癌患者铜绿假单胞菌制剂术中腹腔置药对免疫功能的影响

摘　　要：[目的]评价胃癌手术患者术中腹腔放置铜绿假单胞菌制剂的安全性及对免疫功能的影响。[方法]72例进展期胃癌患者随机分为铜绿假单胞菌制剂腹腔置药组(实验组)和对照组,实验组术毕于原手术部位按0.08～0.12ml/kg放置铜绿假单胞菌注射液,对照组予以单纯手术。全部病例均在术前及术后10、20、30d采集外周静脉血分别测定血常规、肝肾功能,CD19(B细胞)计数水平,免疫球蛋白水平(IgA、IgM和IgG),并观察患者围手术期并发症及不良反应。[结果]实验组围手术期血常规、肝肾功能均在正常值范围内。术后30d,实验组较对照组IgA(P=0.037)、IgG(P=0.044)及CD19计数水平(P=0.043)明显提高。实验组术后10d IgM较术前明显增高(P=0.011)。两组均未出现明显不良反应。[结论]铜绿假单胞菌制剂胃癌术中腹腔给药安全可行,可增强机体体液免疫调节作用。[Purpose] To investigate the effect of pseudomonas aeruginous-mannose sensitive haemagglutination（PA-MSHA） intraperitoneal administration during operation on immunity and its safety in patients with gastric cancer.[Methods] Seventy two cases with advanced gastric cancer were randomly divided into PA-MSHA treatment group（experimental group） and control group.Patients in experimental group received PA-MSHA 0.08~0.12ml per kilogram in the original surgical site at the end of operation,whereas patients in control group received surgery alone.Peripheral venous blood samples were obtained before operation and at the 10th,20th and 30th day after operation,the blood routine examination,liver and kidney function,total B-lymphocyte count（CD19） and the immunoglobulin level were detected.The perioperative complications and toxicity were observed.[Results] The blood routine examination and liver and kidney function were normal in perioperative period.Thirty days after surgery,compared with the control group,the concentration of IgA（P=0.037） and IgG（P=0.044） and the count of CD19 cells（P=0.043） in experimental group increased significantly.The concentration of IgM at day 10 after surgery increased significantly in experimental group.There was no obvious toxicity in both groups.[Conclusion] The intraoperative intraperitoneal administration of PA-MSHA for patients with gastric cancer is safe and feasible,and it can enhance the patients＇ immunity.
