囊性颅咽管瘤立体定向内放疗的长期疗效分析

摘　　要：目的探讨颅咽管瘤内放疗的长期治疗结果。方法对743例立体定向32 P内放疗的囊性颅咽管瘤患者进行长期疗效回顾性分析,术后门诊复查及电话随访10～20年(平均12.2年)。结果 743例颅咽管瘤治疗情况。CT/MR影像复查肿瘤体积变化:肿瘤消失416例(56.0%),明显缩小(>50%)221例(29.7%),无变化65例(8.7%),肿瘤复发41例(5.5%);临床症状:神经功能改善620例(83.4%),无变化73例(9.8%),恶化42例(5.7%),死亡9例(1.2%)。结论临床所见颅咽管瘤多数呈囊实性或囊性,可经此疗法治愈。立体定向内放疗不仅创伤较小,对囊性颅咽管瘤长期疗效也令人满意。Objective To investigate the long-term results in clinical treatment of cystic craniopharyngioma with stereotactic brachytherapy.Methods Clinical data of 743 patients with cystic craniopharyngioma were analyzed retrospectively.All tumors were treated with stereotactic injection of isotope 32P.Results Followed-up 743 patients 10 to 20 years（average 12.2 years） by outpatient service and telephone inquire,tumors disappear in 416 cases（56.0%）,subtotal removal in 221 cases（29.7%）,stable in 65 cases（8.7%）,recurrence in 41 cases（5.5%）.Clinical symptoms improved in 620 cases（83.4%）,stable in 73 cases（9.8%）,worsened in 42（5.7%） and dead in 9 cases（1.2%）.Conclusion Total tumor removal could be achieved in most craniopharyngioma with stereotactic injection of 32P.Since most craniopharyngiomas are cystic,which is conviniant for this therapy.Not only brachytherapy has minimal invasion to the patients,but also has good results for cystic craniopharyngioma during long time.
