多层羊膜移植术治疗深层蚕食性角膜溃疡

摘　　要：目的探讨农村基层医院在角膜供体短缺的情况下深层蚕食性角膜溃疡行多层羊膜移植术的治疗效果。方法回顾分析近3年我院收治的8例（8眼）原发性深层蚕食性角膜溃疡，溃疡面积最小的2mm×3mm，最大的3mm×7mm，达深基质层，病程长3个月至1年，且反复发作，均采取多层羊膜移植术联合常规综合性治疗。结果本组病例术后溃疡面均在1—2周内愈合，外层羊膜2周左右脱落或溶解吸收，内层羊膜贴附紧密，2—3个月后渐渐吸收。随访3个月至1年，全组8例角膜上皮完好，原溃疡处角膜变薄，轻度凹陷，未见复发。结论在不具备角膜板层移植术条件的农村基层医院，多层羊膜移植术是深层蚕食性角膜溃疡较好的治疗手段。Objective To study the therapeutic effect of deep mooren＇ s ulcer line with multilayer amniotic membrane transplantation in the countryside hospital with a lack of donor cornea. Methods Retrospective analysis was made in 8 cases （8 eyes）of the deep Mooren＇ s ulcer in our hospital. The smallest ulcer was 2 mm ×3mm, The biggest was 3 mm ×7 mm, Ulcers were deep in the matrix layer, for three months to one year, And frequent, accepted muhilayer amniotic membrane in transplantation combined regular comprehensive treatment. Results The ulcers in this survey healed smoothly within 2 weeks, Outer amniotic failed off or dissolve in two weeks , Inner amniotic adhesion into the ulcer and gradually dissolved within 2 - 3 months. Followed up for 3 months to 1 year. All 8 cases had integrity of the corneal epithelium, thinning of the original ulcer, with no recurrence. Conclusion Muhilayer amniotic membrane transplanta- tion is an effective method to treat deep Mooren＇ s ulcer in countryside hospital without condition of the lamellar keratoplasty.
