摘  要:目的分析激素受体ER,PR及生长因子VEGF,bFGF在垂体腺瘤中的表达,探讨ER,PR,VEGF,bFGF与垂体腺瘤分型及侵袭性之间的关系。方法取经手术切除且病理诊断为垂体腺瘤、具有完整内分泌资料的患者80例,根据Hardy-Wilson及Knosp分级、分期标准,结合病理活检判断其肿瘤侵袭性,将肿瘤的侵袭程度分为0、1、2、3级,分别为10例、11例、30例和29例。用免疫组化方法分别检测每例肿瘤ER,PR,VEGF和bFGF的表达水平,用RT-PCR检测VEGF和bFGF mRNA的表达。结果 80例患者中ER,PR表达的阳性率分别为62.5%(50/80)和60%(48/80);PRL腺瘤与无功能腺瘤之间ER蛋白表达阳性率的呈显著性差异(P<0.05),PR表达水平与垂体腺瘤分型相关性不显著(P>0.05)。随肿瘤侵袭性程度的增加,VEGF和bFGFmR NA的表达和免疫组化阳性率及分级相应增加。结论 ER的表达水平与PR L型垂体腺瘤有关,其表达水平有可能作为判断PR L型垂体腺瘤发生和预后的指标。生长因子VEGF和bFGF mR NA、蛋白的表达也与垂体腺瘤侵袭性密切相关,有可能作为抑制垂体腺瘤侵袭性的治疗靶标。【Objective】 To explore the expression of estrogen receptor(ER) and progesterone receptor(PR),VEGF and bFGF in pituitary adenoma,and analysis study their correlation with the grouping,invasiveness of the tumors.【Methods】 Eighty patients were collected after surgery from patients after full endocrine preoperative evaluation.All tumors were graded and staged according to Hardy-Wilson and Knosp classification,using magnetic resonance scanning.Immunohistochemical staining was adapted to semi-quantify the expression of ER,PR,VEGF and bFGF.RT-PCR was applied to detect mRNA expression of VEGF and bFGF.【Results】 The prevalence of ER and PR were 62.5% and 60% in pituitary adenoma respectively.Significant negative correlation could be seen between the expression of ER and PRL type of pituitary adenoma,but there was no clear relationship could be found between the expression of PR and the grouping of pituitary adenoma.Moreover,the invasion of pituitary adenoma was positive correlated to the mRNA and protein expression level of VEGF and bFGF.【Conclusion】 The expression of ER may be considered as the markers to indicate diagnosis and prognosis of pituitary adenoma.And VEGF and bFGF may be considered as the markers to indicate invasion of pituitary adenoma and therapeutic targets.
