摘　　要：目的对上海某社区精神病人进行社区干预并评价干预效果,探讨提高精神病患者健康状况的有效措施。方法整群抽取上海田林社区,以该社区2007年4月筛查出的精神病患者322例为研究对象,对其提供咨询、心理干预、个案服务、免费提供抗精神病药物,同时采取有目的的随访、管理,3年后对干预前后患者康复措施落实率、精神卫生知晓率、患者依从率、病情改善率、社会参与率、肇事肇祸率等指标进行对比分析。结果社区干预使康复措施的落实率由51.9%上升到98.8%(P﹤0.001);知晓率由44.1%上升到86.3%(P﹤0.001);患者依从率从干预前的41.0%上升到干预后的84.8%(P﹤0.001);病情控制率由24.2%上升到86.0%(P﹤0.001);社会参与率由39.8%上升到64.0%(P﹤0.001);精神病人肇事肇祸率由3.4%下降到0.6%(P﹤0.001)。结论社区干预能显著提高精神病患者的健康状况同时给社会带来稳定。OBJECTIVE To research the Shanghai community intervention for patients and evaluate intervention effects, and investigate the effective measures of improving the health status of the mentally ill. METHODS The examination of mental diseases was carried out among the community residents, and 322 patients with mental illness were clustered collected from Tianlin District of Shanghai in April of 2007. All the patients were provided with free consultation, psychological intervention, casework service and the anti-neurosis medicines; Meanwhile, some measures such as intentional follow-ups and management were taken. Three years later, a comparative analysis which aimed to analyze the indicators of the patients’ compliance, the rates of practical recovery measures, mental health awareness, disease improvement, social participation and accidents before and after the intervention were completed. RESULTS The rate of practical rehabilitation measures was at the range of 44.1% to 86.3% （P ﹤0.001）; The rate of disease conformity was at the range of 24.2% to 86.0% （P﹤0.001）; The rate of patients’ compliance increased from 41.0% to 84.8% （P﹤0.001）; The rate of social participation was at the range of 39.8% to 64.0% （P﹤0.001）; The rate of causing trouble was decreased from 3.4% to 0.6% （P﹤0.001）. CONCLUSION Community intervention trials can obviously improve the mental patients’ health, which is good for the whole society.
