不同下斜肌减弱术治疗V型斜视临床比较研究

摘　　要：目的观察并比较不同下斜肌减弱术治疗V型斜视临床效果及安全性。方法选取某院2008年3月～2010年7月收治下斜肌亢进所致V型斜视患者180例,随机分为3组,分别采用下斜肌切断术、下斜肌后徙术及下斜肌部分截除术治疗;比较3组患者V症、下斜肌功能治疗有效率及术后黏连发生率。结果下斜肌切断术组、下斜肌后徙术组及下斜肌部分截除术组患者V症治疗有效率组间比较差异无统计学意义(χ2=0.934,P﹥0.05);3组患者下斜肌功能治疗有效率组间比较差异亦无统计学意义(χ2=0.715,P﹥0.05);但下斜肌后徙术组患者术后组织黏连发生率明显低于下斜肌切断术组及下斜肌部分截除术组,组间比较差异有统计学意义(P﹤0.05)。结论下斜肌切断术、下斜肌后徙术及下斜肌部分截除术治疗V型斜视临床效果相当,但是下斜肌后徙术术后患者黏连发生率较低,可作为V型斜视临床治疗首选术式。OBJECTIVE To observe and compare the therapeutic effect and security of different inferior oblique weakening surgery for patients with V-type strabismus. METHODS 180 patients with V-type strabismus were chose in the period from March 2008 to July 2010 in our hospital, and randomly divided into three groups including inferior oblique transection group （60 patients）, inferior oblique recession group （60 patients） and inferior oblique section cutting group （60 patients）. The treatment efficiency of V-type symptoms, inferior oblique function and the incidence of postoperative adhesions among three groups were compared. RESULTS There was no significant difference in the treatment efficiency of V-type symptoms and inferior oblique function among inferior oblique transection group, inferior oblique recession group and inferior oblique section cutting group （P﹥0.05）. But the incidence of postoperative adhesions of inferior oblique recession group was significantly lower than inferior oblique transection group and inferior oblique section cutting group （P﹤0.05）. CONCLUSION The therapeutic effects of inferior oblique transection group, inferior oblique recession group and inferior oblique section cutting group are close to each other; but the patients with inferior oblique recession could gain lower incidence of postoperative adhesions, and this method can be used as the preferred surgical methods to V-type strabismus.
