肺结核合并肺癌的临床及影像学特点分析

摘　　要：目的总结概括肺结核合并肺癌患者的临床特征,并分析CT影像学特点,从而提高临床诊断率。方法收集我院2009年4月～2010年4月诊断的46例肺结核合并肺癌患者,回顾性分析患者的临床资料。结果患者的主要临床特征为消瘦、气喘、痰血、胸痛、盗汗、低热等。46例患者中,12例患者的肺癌部位为出现肺结核的肺段、肺叶,19例患者的CT影像具有明显的中央型肺癌特点,15例患者的CT影像具有明显的周围型肺癌特点,2例患者的CT影像不能诊断为肺癌。大部分患者的CT影像学特点明显。结论临床诊断中常有肺结核合并肺癌的患者,肺癌影像无典型特点的患者不易被诊断出来,针对进展期和陈旧性肺结核CT影像异常的患者应检查是否合并肺癌并及早确诊。Objective To summarize the clinical characteristics of pulmonary tuberculosis complicated by lung cancer and analyze the CT imageological characteristics, and thus improve the clinical diagnosis rate. Methods The clinical data of 46 patients with pulmonary tuberculosis complicated by lung cancer diagnosed in our hospital from April 2009 to April 2010 were analyzed retrospectively. Results The main clinical characteristics of patients were weight loss, asthma, hemoptysis, chest pain, night sweats, fever, etc. Of the 46 patients, 12 patients＇ sites of lung cancer were lung segments and lobes with pulmonary tuberculosis. Nineteen patients＇ CT images had obvious characteristics of central lung cancer. Fifteen patients＇ CT images had obvious characteristics of peripheral lung cancer. Two patients＇ CT images couldn＇t be diagnosed as lung cancer. Most patients＇ CT images had obvious characteristics. Conclusion Pulmonary tuberculosis complicated by lung cancer is common in clinical diagnosis. For patients with atypical imaging characteristics, lung cancer can＇t be easily di- agnosed. For patients with progressive and chronic pulmonary tuberculosis who have abnormal CT images, it is necessary to check if there is lung cancer and diagnose in an early stage.
