留置双胃管治疗食管癌术后吻合口瘘的效果分析

摘　　要：目的探讨食管癌术后吻合口瘘应用双胃管经鼻肠内营养的临床治疗效果。方法近7年来,将20例食管癌术后吻合口瘘患者随机分为治疗组(A组)10例和对照组(B组)10例,均行食管癌根治术,A组在吻合口瘘后应用双胃管经鼻肠内营养术(EN);B组吻合口瘘后经外周静脉行肠外营养(PN)。观察两组食管癌术后吻合口瘘的治疗效果,平均费用、平均住院费用等。结果 A组平均治愈率高于B组,而平均住院费用低于B组,两组比较差异有统计学意义(P<0.05)。结论食管癌术后吻合口瘘应用双胃管经鼻肠内营养(EN)可明显改善患者术后吻合口瘘营养状况,双胃管在加强引流的同时,进行胃肠减压,双胃管形成一种循环的引流作用,既能够起到引流作用,又能够为肠道提高营养,是治疗吻合口瘘一种完美的结合,是目前实施营养治疗比较理想的方法,利于患者康复以及改善生存质量,减少严重并发症发生,住院时间缩短并降低费用。Objective To investigate the clinical effection of Nasal Enteral Nutrition by double gastric tube on anastomotic fistula in esophageal carcinoma.Methods Nearly 7 years,20 cases of patients with anastomotic fistula after operation were randomly divided into treatment group（group A,n=10） and control group（group B,n=10）.Group A applicative Nasal Enteral Nutrition line of total parenteral nutrition by double gastric tube（EN）,while Group B applicative perpheral vonous line parenteral nutrition.Results The treatent effect,the verage cost,the hospitalization cost and so on were incidence of complications and hospitalization costs in Group A signisficantly lower than Group B（P0.05）.Conclusion The patients with anastomotic fistula in postoperotive esophageal carcinoma by double tube nasal enter nutrition make signnifcant improvement in the nutritional status.Double Gastric Tube enchanced draining meanwhile it can gastrointerinal decompression.Double Gastric Tube not only formed a cycle of drainage role which can be up to drainage,but also improve intestinal nutrition.Double Gastric Tube is a perfect combination in treating anastomosis mouth,is a comparision ideal in nutrition treatment.Besides it can conducive to patients and improve survival quality,reduce serious complications,short hospitalization and reduce costs.
