氩离子凝固术联合埃索美拉唑治疗Barrett食管疗效观察

摘　　要：目的对氩离子凝固术联合埃索美拉唑治疗Barrett食管(BE)近期疗效进行评价。方法选择40例BE患者,采用随机数字表法分为埃索美拉唑治疗组(单纯药物组)和氩离子凝固术联合埃索美拉唑治疗组(联合治疗组),2组基线资料均衡。3、6、12个月分别随访,对其临床症状缓解情况和消除与复发情况进行评价。结果 12个月联合治疗组治疗满意率明显高于单纯药物组(85.0%vs55.0%,P<0.05)。在联合治疗组中,氩离子凝固术对短段BE治愈率达100%,2例长段BE在6个月时复发;未出现严重溃疡、出血和穿孔等并发症。单纯药物组未见病灶消除。结论氩离子凝固术联合埃索美拉唑治疗BE是安全有效的治疗方法。Objective To assess the short-term efiect of argon plasma coagulation （ APC ） combined with esome- prazole for treatment of Barrett$ esophagus （ BE）. Methods Forty patients with BE were randomly divided into age-, gender, and morphological classification-matched 2 groups using number table （n = 20 each） ：simple drug group and combination therapy group. In simple drug group, esomeprazole was administrated （20 mg bid for 4 weeks, then 20 mg qd for 8 weeks）. In combination therapy group, APC （ by APC 300 apparatus） combined with esomeprazole （ditto） therapy was used. The follow-up at 3-,6-and 12-month was performed for evaluating remittent condition of clinical symptom and the condition of elimination and recurrence of foci. Results At the 12-month follow-up, the treatment satisfaction rate in combination therapy group was significantly higher than that in simple drug group （ 85.0% vs 55.0% , P 〈0. 05 ）. In combination therapy group, the cure rate of short-segment BE treated APC reached a percentage of 100% ;2 cases of long-segment BE recurred at 6 months; complications such as severe ulcer, bleeding and perforation didnt occur. In simple drug group,it was not found that the foei were eliminated. Conclusion APC combined with esomeprazole is a safe and effective method for treatment of BE.
