促红细胞生成素对急性脑梗塞患者NSE及炎性细胞因子的影响

摘　　要：目的:观察重组人促红细胞生成素(rHu-EPO)对急性脑梗塞患者的血清NSE及炎症细胞因子:超敏C反应蛋白(hs-CPR)、白介素-10(IL-10)的影响。方法:选取住院的急性脑梗塞患者80例,随机分为治疗组(40例)、对照组(40例),均按急性脑梗塞临床路径的规范给予治疗。治疗组在正规治疗的基础上给予rHu-EPO6000U/次,每2天一次,共3次。所有患者均于入院时及治疗后第14天清晨抽取静脉血,采用双抗体夹心酶联免疫法(ELSIA)检测NSE、hs-CPR、IL-10水平。结果:⑴两组患者入院时的NSE水平无统计学差异,治疗后均下降(P<0.05);治疗后治疗组的NSE水平明显低于于对照组(P<0.05),表明治疗组患者的预后要优于对照组。⑵两组患者入院时的hs-CPR、IL-10水平无统计学差异;治疗后第14天hs-CPR、IL-10的水平均下降,而治疗组hs-CPR、IL-10水平降低更明显(两组间比较均P<0.05)。结论:促红细胞生成素对急性脑梗塞患者具有神经保护作用,能改善患者的预后。其神经保护机制是通过抑制脑梗塞炎症反应及免疫损伤,减少神经细胞凋亡,而起到神经保护、改善预后的作用。Objective：To observe the effect of recombinant human erythropoietin （rHu -EPO）on the levels of serum neu- ron -specific enolase（NSE） and inflammatory cytokine such as high sensitivity C -reaction protein（ hs -CPR） ,interlerkin- 10（IL-10） in acute cerebral infarction patients. Methods：Eighty cases were randomly divided into two groups：treatment group（40 cases）and control group（40 cases）, all patients were treated by routine clinical therapy. But treatment group were provided extra drug that is rHu - EPO. ELS!A was used to detect the levels of NSE,hs - CPR and IL - 10 on admission,14 days after admission. Results： NSE,hs -CPR and IL- 10 levels in the two groups have no significant difference on admission. But there were found that them all decreased significantly in both treatment group and control group, especially in treat- ment group （P 〈 0, 05 ）. That means the therapeutic effect of treatment group were superior to those of control group. ConclusloniThe therapy of using rHu- EPO not only significantly restrain the inflammatory response,immune injury, but significantly reduce neuron apoptosis, and thus can protect Nerves of acute cerebral infarction patients and improve patient prognosis.
