支持垫规范康复体位护理在脑卒中卧床患者康复中的应用

摘　　要：目的:探讨康复体位护理对脑卒中卧床患者功能恢复及防止各种并发症的作用。方法:将86例脑卒中偏瘫卧床患者随机分为支持垫组和对照组,各43例。对照组给予康复治疗联合传统护理,支持垫组实施康复治疗+康复体位护理(使用体位支持垫),干预后3周、6周分别采用Barthel指数(BI)、简化Fugl-Meyer运动功能量表进行评定,并统计2组并发症发生率。结果:2组患者入选时BI、简化Fugl-Meyer运动功能评分差异均无统计学意义;干预3周和6周后,支持垫组简化Fugl-Meyer运动功能评分及BI评分提高分均显著高于对照组(P<0.01)。并发症发生率干预3周后,2组患者差异均无统计意义(P>0.05);6周后,支持垫组发生率较对照组均显著降低(P<0.01)。结论:体位支持垫有助于脑卒中偏瘫卧床患者运动功能的恢复,同时减少各种并发症。Objective ：To investigate the effect of the rehabilitation postural care on promoting the function recovery and preventing complications in the early stage of stroke patients. Methods： Eight-six patients with stroke were chosen and randomly divided into postural care group and control group（43 cases each group）. The patients of the postural care group received the rehabilitation therapy and the standardized postural care（ using posture supporting cushion） ,while the patients of the control group received the rehabilitation therapy and the traditional nursing. All patients were assessed with Barthel Index（BI） and Modified Fugl-Meyer Motor Function Scale at the 3rd week and the 6th week after therapy. The incidence rate of complications of two groups was measured. Results： No significant differences of BI and Modified Fugl-Meyer Motor Function Scale of two groups were found at the recruitment. After 3 weeks, the BI scores in the postural care group was better than the control group（P 〈0.05 ） ,and the improved scores of BI and Modified Fugl-Meyer Motor Function Scale of the postural care group were significantly higher than that of the control group in the 6th week. The incidence rate of complications of two groups showed no difference during 3 weeks,while after 6 weeks,the incidence rate of the complications in the postural care group decreased significantly compared with the control group（ P 〈 0.01 ）. Conclnsions： Rehabilitation postural care can promote the recovery of the motor function of the stroke patients and decrease the incidence rate of complications.
