超声冲洗联合甘油氢氧化钙治疗窦道型慢性根尖周炎的临床疗效观察

摘　　要：目的 评价根管超声冲洗和甘油氢氧化钙糊剂作为管内封药运用于有窦型慢性根尖周炎的临床效果.方法 对150例有窦型慢性根尖周病患牙进行根管治疗,按就诊顺序随机分组,试验组1（50例）采用超声冲洗和甘油氢氧化钙糊剂根管内封药,试验组2（50例）采用注射冲洗和甘油氢氧化钙糊剂根管内封药,对照组（50例）采用注射冲洗和甲醛甲酚根管内封药,比较3组根管冲洗后细菌检出率,根管封药术后反应和根充1年后的临床疗效.结果 试验组1根管内细菌检出率明显低于试验组2和对照组（P＜0.05）；试验组1和2根管封药术后疼痛的发生率明显低于对照组,一次性封药有效率[92.0％（46/50）,80.0％ （40/50）]明显高于对照组[64.0％ （32/50）],试验组1在术后1年后根管治疗成功病例的比例[74.4％ （32/43）]明显高于高于试验组2[56.8％ （25/44）]和对照组[45.5％ （20/44）],差异有统计学意义（P＜0.05）.结论 超声根管冲洗能有效减少感染根管内细菌及毒素,联合氢氧化钙作为根管内封药有利于减少术后不良反和封药次数,有利于提高根管治疗的成功率.Objective To observe the clinical effect of ultrasonic irrigation combined with glycerine-calcium hydroxide paste used in root canal therapy on periapical abscess with sinuses. Methods One hundred and fifty teeth of chronic periapical abscess with sinuses were randomly selected and divided into three groups. Root canals of the test group 1 were irrigated by ultrasonic technique and sterilized with glycerine-calcium hydroxide paste. Root ca- nals of the test group 2 were irrigated by syringes and sterilized with glycerine-calcium hydroxide paste, while those root canals of control group were irrigated by syringes and sterilized with formaldelhyde cresol （FC）. The bacterial amount of three groups was checked; clinical observation and contrast were analyzed among these three groups. Re- suits The detection rate and amount of bacteria in the root canals of the test group 1 were significantly lower than those in the test group 2 and the control group. Postoperative pain incidence in two test groups was significantly lower than that in the control group. While the efficacy of one block therapy in two groups [ 92.0% （46/50）, 80. 0% （40/ 50） ~ was signally higher than that in the control group [ 64. 0% （32/50） ]. The ratio of successful root canal therapy cases after 1 year in the test group 1[74. 4% （ 32/43 ） ] was significantly higher than that in the test group 2 and the control group[56. 8% （25/44） ,45.5% （20/44） 1. Conclusion Ultrasonic irrigation of the root canal and glycerine-calcium hydroxide paste used in root canal disinfection can be an useful technique in root canal therapy.
