套筒冠固位加中空式赝复体修复老年人肿瘤术后单侧上颌骨缺损  

Clinical effect of Telescopic Crown Retainer and Hollow Prosthetic Appliance on restoring Maxillary Defect after Neoplasm Surgery for the Aged Patient

摘　　要：目的:随访观察1-5年套筒冠固位加中空式赝复体修复老年人肿瘤术后的单侧上颌骨缺损的效果。方法:选择2005年1月至2010年1月门诊60岁以上老年人肿瘤术后的单侧上颌骨缺损伴牙槽骨缺失,口鼻相通伴发音差,健侧余留牙严重磨耗致咬合间隙小。采用套筒冠固位加中空式赝复体修复26例,随访观察1-5年复查修复体的密合性,美观与舒适,固位及稳定性,咬合关系和咀嚼功能,基牙牙周组织等。结果:随访观察1-5年套筒冠固位加中空式赝复体修复后效果:满意61.5%,基本满意34.6%,差3.8%,总满意率96.1%。结论:套筒冠固位加中空式赝复体修复老年人肿瘤术后的单侧上颌骨缺损效果较满意。Objective： To observe the clinical effect of telescopic crown retainer and hollow prosthesis appliance on repairing unilateral maxillary defect after neoplasm surgery for 1- 5 years by follow-up visiting on aged patients. Methods： Twenty-six cases of the aged patients who suffered from unilateral maxillary defect, alveolar bone defect, poor pronunciation caused by mouth and nose connection, and inadequate occlusion clearance caused by severe tooth abrasion after neoplasm surgery were selected. All individuals were implemented with telescopic crown retainer plus hollow prosthetic appliance. After a follow-up period of 1 to 5 years, adaptation, appearance and fitness, maintenance and stability, occlusion and masticatory fimction as well as periodontal condition and so on were observed and detected. Results： It has been shown that the total satisfaction rate with the treatment of telescopic crown retainer plus hollow prosthetic appliance was 61.5%, adequate satisfactory rate was 34.6% and dissatisfactory rate was 3.8%, with general satisfactory rate of 96.1%. Conclusion： The application of telescopic crown retainer plus hollow prosthetic appliance was shown to be satisfactory in repairing unilateral maxillary defect after neoplasm surgery of the aged patients.
