摘 要:目的探讨重组人肠三叶因子(rhITF)对烧伤后肠源性高代谢的作用及期机制。方法采用30%体表面积Ⅲ度烧伤小鼠模型,将72只BALB/c小鼠随机分为正常对照组(C组,8只)、烧伤对照组(B组,每个时相点8只)和rhITF治疗组(ITF组,每个时相点8只)。观察伤前及烧伤后1、3、5、7d小鼠静息能量代谢率(REE)的变化,同时检测伤后血浆内毒素(LPS)、肿瘤坏死因子α(TNFα)、白细胞介素-1(IL-1)和白细胞介素-6(IL-6)的含量。结果两组烧伤小鼠的REE、TNFα、LPS、IL-1和IL-6水平均明显高于烧伤前水平(P<0.05),两组比较,ITF组小鼠REE明显低于B组,平均降幅达25%左右。与之对应,血浆LPS、TNF-α、IL-1和IL-6含量也明显低于B组(P<0.05)。结论 rhITF能减轻烧伤后肠道受损程度,降低烧伤小鼠血中炎症介质水平,从而减轻烧伤后肠源性高代谢。Objective To study the effects of recombinant human intestinal trefoil factor (rhlTF) on enterogenous hyperme- taholism after severe burn injury and its possible mechanisms. Methods The mouse model of 30G body surface area third degree burns was established. 72 BALB/c mice were randomly divided into three groups:normal control(C, 8 cases), burned control(B, 8 cases at each time point) and rhITF treatment(ITF, 8 cases at each time point) groups. Resting energy expenditure(REE), the level of plasma tumor necrosis factor alpha( TNFa), lipopolysaccharide (LPS), interleukin-1 ( IL-1 ) and interleukin-6 (IL-6) were determined before burn and on postburn 1,3,5,7 d. Results After burn injury,the levels of REE, plasma TNFa, I.PS,IL-1 and IL-6 in the group B and ITF was significant higher than those before burn(P〈0.05). Compared with the group B, the level of REE in the group ITF were remarkable decreased with the average decreasing amplitude of 25%,and the plasma TNFα, LPS, IL-1 and IL-6 levels were also lower than those in the group B(P〈0.05). Conclusion rhITF treatment could abate the degree of intestine injury,decrease the inflammatory mediators in blood and reduce the enterogenous hypermitabolism.
