摘  要:目的:观察博莱霉素(bleomycin,BLM)致大鼠肺纤维化中核因子κB(nuclearfactorκB,NFκB)活性和表达及红霉素治疗对其的影响。方法:用Wistar雄性大鼠一次性气管内注射BLM(每千克体重5mg)制成肺纤维化模型,治疗组于BLM灌注后每日胃内注入红霉素(每千克体重100mg),对照组气管内、胃内注射生理盐水。3组动物于气管内灌注后第4、7和28天处死,用电泳迁移分析法测定肺泡巨噬细胞(alveolarmacrophages,AM)的NFκB活性,用免疫组织化学法检测肺组织NFκB成分p65表达。结果:第4和7天时BLM组肺AM的NFκB活性和肺组织p65表达较对照组显著增高,红霉素治疗组NFκB活性和p65表达较BLM组显著降低,第28天时3组动物NFκB活性差异没有显著性。红霉素治疗组伴有肺组织病理的改善:早期炎性细胞浸润减少,晚期纤维化程度减轻。结论:NFκB活性和表达增加可能与肺纤维化发病有关系,红霉素可能通过抑制NFκB活性起抗炎、抗纤维化作用。Objective: To investigate the activity and expression of nuclear factor-κB (NF-κB) in bleomycin induced pulmonary fibrosis in rats and the effects of erythromycin on NF-κB in this model. Methods: Pulmonary fibrosis model was induced by intratracheal instillation of single dose bleomycin(BLM) 5 mg·kg -1 in male Wistar rats. Erythromycin(EM) 100 mg·kg -1 was administered daily by oral in the therapeutic groups. The animals were killed on the 4th, 7th and the 28th day separately. The NF-κB activity in alveolar macrophages (AM) was determined using an electrophoretic mobility shift assay (EMSA). The expression of p65 in NF-κB components of lung tissue was detectable by immunostaining. Results: From day 4 to day 7, the activity of NF-κB in AM and the expression of p65 in lung tissue were significantly increased in BLM groups compared with those of control groups, while the NF-κB activity and the p65 expression were significantly decreased in EM groups compared with those of BLM groups. On the 28th day, there was no significant difference of NF κB activity among the three groups. Pathologically, EM decreased exudation of inflammatory cells in the early response as well as the degree of fibrosis in the late stage. Conclusion: These results suggest that NF-κB is involved in the process of BLM induced pulmonary fibrosis in rats. EM has inhibitory effects on alveolitis and pulmonary fibrosis in this model possibly by depressing NF-κB activity.
