摘 要:目的比较原发性鼻腔非霍奇金淋巴瘤(non-Hodgkin’s lymphomas,NHL)和慢性鼻-鼻窦炎中Epstein-Bar(EB)病毒缺失型潜伏膜蛋白1(latent membrane protein 1,LMP1)基因的表达,探讨原发性鼻腔非霍奇金淋巴瘤LMP1缺失型与预后的关系。方法采用免疫组织化学方法检测55例原发性鼻腔NHL和25例慢性鼻-鼻窦炎LMP1,采用聚合酶链式反应(polymersechain reaction,PCR)检测LMP1基因,结合随访资料分析LMP1缺失型鼻腔NHL与预后的关系。结果鼻腔NHL中免疫组化LMP1蛋白阳性率为29/55(53%),LMP1基因阳性率为33/55(60%),其中缺失型LMP1基因者26/33(78.8%);慢性鼻-鼻窦炎中LMP1基因阳性率为14/25(56%),其中缺失型LMP1基因者5/14(35.7%)。鼻腔NHL患者和慢性鼻—鼻窦炎患者缺失型LMP1基因的表达差异有统计学意义(P<0.05)。LMP1基因阳性的鼻腔NHL患者5年总生存率为52.4%,其中LMP1缺失型和LMP1野生型鼻腔NHL的5年生存率分别为36.0%和85.7%,二者生存曲线差异有统计学意义,P值为0.04(log-rank检验)。结论提示缺失型LMP1在原发性鼻腔NHL的发病和病情进展中可能起重要作用。Objective: To compare the expression rates of Epstein -Barr(EB) virus deleted latent membrane protein 1 ( LMP1 ) in primary nasal non - Hodgldn~ lymphomas with those in chronic rhinosinusitis, and analyze the prognostic sig- nificance of LMP1 deletion in primary nasal non - Hodgking lymphomas (NHL). Methods: The expressions of latent mem- brane protein 1 (LMP1) were detected by immunohistochemistry(IHC) and PCR respectively in 55 cases of primary nasal NHL and 25 cases of chronic rhinosinusitis. Reletionship of deleted LMP1 gene to prognosis of primary nasal NHL was ana- lyzed. Results: LMP1 protein was expressed in 29 of 55 (53%) nasal NHL by IHC. 33 of 55 (60%) NHL was positive for clonal LMP1 gene rearrangement, and 26 of 33 (78.8%) contained deleted LMP1 gene. 14 of 25 (56%) chronic rhino- sinusitis was positive for clonal LMP1 gene rearrangement, and 5 of 14(35.7% ) contained deleted LMP1 gene. Ther posi- tive rate of deleted LMP1 gene in primary nasal NHL was significantly higer than that in chronic rhinosinusitis (P 〈 0.05 ). The 5 - year overall survival rate for nasal NHL patients who contained LMP1 gene was 52.4%. The 5 - year survival rates were 36.0% for nasal NHL patients who contained deleted LMP1 gene, and 85.7% for those who contained wide - type LMP1 gene. Of the primary nasal NHL, patients who contained deleted LMP1 gene seemed to carry worse prognosis than those who contained wide- type LMP1 gene, and the significance probability was 0.04 (log -rank). Conclusion: The re- suits suggest that deleted LMP1 may play an important role in the pathogenesis and progress of primary nasal NHL.
