摘 要:目的评估癫痫儿童的智力水平和结构,探讨临床特征因素对癫痫儿童智力水平的影响。方法选用中国修订韦氏儿童智力量表中的相似性、数字广度、木块图和编码测验对53例癫痫儿童和54例正常儿童进行测查及对比分析,多因素分析方法探讨癫痫儿童智力水平与临床特征因素的关系。结果病例组儿童智力水平在正常范围内,但总智商及相似性、数字广度、木块图、编码测验得分均低于对照组(109.97±10.72)分vs(118.89±9.05)分,(13.21±2.51)分vs(14.39±2.45)分,(11.04±2.83)分vs(12.56±2.23)分,(10.47±2.77)分vs(12.61±2.54)分,(10.45±3.15)分vs(13.33±2.97)分(P<0.05或<0.01);继发全面性发作、全面性发作、脑电图癫痫样放电范围、发作持续时间及用药数量是智商下降的危险因素。结论癫痫儿童智力水平和结构存在一定程度的损害,发作类型、脑电图癫痫样放电范围、发作持续时间及用药数量是影响癫痫儿童智力水平和结构的主要临床特征因素。Objective Evaluating the differences of intellectual level and intellectual structure between children with epilepsy and normal children; Exploring the effects of clinical characteristics on intelligence of children with epilepsy. Methods Fifty-three children with epilepsy and 54 normal children were evaluated with similarities test,digit span test,block design test and coding test in Chinese Wechsler Intelligence Scale for Children(C-WISC) to explore the relation of the intelligence of children with epilepsy and clinical characteristics by multiple linear regression analysis. Results The level of intelligence of children with epilepsy was normal,but the scores of FIQ and similarities test,digit span test, block design test and coding test of children with epilepsy were all significantly lower than those of control group,(109.97±10.72) marks vs (118.89±9.05) marks, (13.21±2.51) marks vs (14.39±2.45) marks,(11. 04± 2.83) marks vs (12.56±2.23) marks,(10.47±2.77) marks vs (12.61±2.54) marks,(10.45±3.15) marks vs (13.33±2.97) marks (all P 〈0.05); factors that entering into FIQ equations were secondary generalized seizures or not,generalized seizures or not, scopes of EEG discharges, duration of attack and quantity of drug, and all the independent variables were negatively correlated with FIQ. Conclusion This study showed that intelligence level and structure of children with epilepsy were damaged in certain degree. Seizure type, scopes of EEG discharges, seizure duration and quantity of drug were the major clinical characteristics that affect the intellectual level and intellectual structure of children with epilepsy.
