摘  要:目的 :探讨胆囊结石、胆石伴梗阻性黄疸时 ET- 1、NO变化及其临床意义。方法 :前瞻性比较观测正常人、胆囊结石、胆石并黄疸病员血液 NO、ET- 1及肝功能指标。结果 :胆石并黄疸组 :NO、白蛋白、球蛋白、AL T、血清胆红素和另两组比较 ,有显著差别 (P<0 .0 5 ) ;NO与 ET- 1、AL T呈负相关 (γET=- 0 .6 17,P<0 .0 1;γAL T=- 0 .319,P<0 .0 5 )。胆囊结石组 :ET- 1和 NO均高于正常对照组 (P<0 .0 5 )。结论 :胆石病员NO高于正常对照组 ,提示 NO可能通过松弛胆囊平滑肌 ,在胆囊功能紊乱的发病机制中起一定作用。梗阻性黄疸时 NO/ ET- 1的原有平衡关系向 NO一侧倾斜将不利于止血 ,这可能是一些病员凝血因子并不缺乏 。Objective:It's unclear the relationship in serum nitric oxide(NO),endothelin 1(ET 1) and hepatic function between patients with gallstone.Methods:A compare was made in serum albumin,globulin,ALT and NO,ET 1 between group A(20 control),group B(35cases with GS) and group C(54 cases with CBDS).Results:There were notable difference in serum bilirubins,NO,albumin,globulin,and ALT between group C and group A and B(P<0.05);plasma ET 1 level in group B was higher than other tow groups(P<0.05),and NO in group B was higher than group A but lower than group C.A negetive correlation between NO and ET 1 were found in group C(γ=-0.617,P<0.01)and in group B(γ=-0.478,P<0.01).Conclusion:It's suggested that NO possibly inducing gallbladder dysfunction passes through relaxation gall bladder smooth muscle;and that the imbalance of NO/ET 1 may be involved in bleeding tendency in patients with obstruct jaundice.
