摘 要:目的:探讨卵巢肿瘤供血动脉在多层螺旋CT血管造影(MSCTA)中的显影特点。方法:搜集卵巢肿瘤行腹盆部CT增强扫描患者104例122侧(原发肿瘤72例79侧,转移肿瘤32例43侧),根据肿瘤直径大小进行分组,A组≤4.0cm(28侧),B组3~5cm(50侧),C组≥5.0cm(44侧),采用容积再现(VR)血管生长技术(AV)进行肿瘤供血动脉重建,分析原发性与转移性、不同直径肿瘤的供血动脉显影特点。结果:122侧卵巢肿瘤总的供血动脉显影率为89.3%(109/122)。79侧原发性和43侧转移性卵巢肿瘤供血动脉显影率分别为88.6%(70/79)、90.7%(39/43),两者比较差异无统计学意义(P>0.05),OA、OB显影率分别为77.9%(95/122)、53.3%(65/122),两者比较差异有统计学意义(P<0.05),A组、B组、C组中,OA显影率分别为53.6%、80.0%、90.9%,OB显影率分别为28.6%、38.0%、86.4%。结论:MSCTA能清楚显示原发性和转移性卵巢肿瘤的供血动脉及其特点,随着肿瘤增大,供血动脉显影率逐渐增高。Objective:To investigate the developing characteristics of multi-slice spiral CT angiography (MSCTA) in the feeding artery of ovarian tumors. Methods: 104 patients with oravian tumors with a total of 122 sides (72 cases of 79 prima- ry side, the transfer of 32 patients with 43 sides) were subjected to the enhanced abdominal pelvic CT scan. Cases were di- vided into two groups, with ≤4.0 cm in diameter (28 sides) in group A, and 〉4.0 cm (64 sides) in group B. Volume rendering (VR) technology based on the growth of blood vessels (AV) was used ,for tumor-feeding artery reconstruction to analyze the imaging characteristics by comparing primary and metastatic tumors, and tumors with different diameters. Re- sulls^Visualization ratio of overall 122 patients with ovarian tumors artery was 89.3% (109/122). Visualization ratio of 79 original and 43 metastatic feeding arteries of ovarian tumor were 88.6%(70/79) and 90.7% (39/43) respectively, with no significant statistic difference ( P 〉 0, 05). Visualization ratio of OA and OB was 77.9% (95/122) and 53.3% (65/ 122) respectively, with significant statistic difference ( P 〈0.05). Visualization ratio of OA in group A (63. 8%) VS group B (90.6%), and OB in group A (22.4%) VS group B (48.4%) respectively, were both with significant statistic difference ( P 〈0. 05). Conclusion: The feeding arteries and blood supply features of the primary or metastic ovarian tumor could be demonstrated by CTA, and the visualization increased as the size of the tumor increased.
