胶质瘤的MRI环形强化特征与病理分析

摘　　要：目的分析胶质瘤(Glioma)的MRI环形强化特征并与病理学表现对照研究。方法回顾性分析76例经手术病理证实的胶质瘤的MRI环形强化特征及其病理学表现。结果 76例胶质瘤中有55例增强扫描呈厚薄不一的不规则环状、花环状、花瓣状强化,病灶的边缘可见星芒状强化,以白质侧为著,多形性胶质母细胞瘤可见壁结节及瘤周结节。病理学示瘤细胞密集,异型性明显,胶质母细胞瘤细胞高度间变、不成熟,伴有出血、坏死明显。边缘可见新生血管结构不良,血管内皮细胞明显增生。结论胶质瘤的MRI环状强化表现与瘤体内的病理改变密切相关,认真分析其特征有助于术前明确诊断。Objective To evaluate the MR imaging features and the pathologic of glioma multiforme. Methods The MR imaging features of 76 glioma lesions confirmed by pathology were reviewed retrospectively and correlated with pathologic findings. Results Fifty-five cases of 76 cases showed irregular ring like or petal like enhancement with varied thickness after contrast on MRI. Star-like rays enhancement appeared on the edge and intra mural nodules could also be found in glio- blastoma tumors. Pathologic histologic characteristics included tumor cells distributed diffusely with marked nuclear atyp ia, muir/nucleated giant cells as well as necrosis and hemorrhage. Vascular proliferation was observed in all glioblastomas. Conclusion The imaging and pathology manifestations of glioma mutiforme were closely correlated. The MRI features was helpful for the pre-surgical diagnosis.
