摘  要:目的比较577nm阈下微脉冲激光与传统激光治疗糖尿病黄斑水肿(DME)的疗效。方法临床病例对照研究。对2011年4-11月在山西省眼科医院经眼科常规检查、FFA及OCT确诊的28例44只眼DME患者纳入此研究。按患者激光号末尾数,用随机数字表法分成两组进行577nm阈下微脉冲激光组(21只眼)及传统激光组(23只眼)治疗。比较两组术后1-2月、3-5月最佳矫正视力(BC-VA)及中心凹视网膜厚度(CMT)的改变。结果微脉冲组和传统组在术前、术后1-2月、3-5月BC—VA差异无统计学意义(z=-0.155、-0.624、-0.258,P:0.877、0.532、0.817,P〉0.05)。两组在术前CMT差异无统计学意义(z=-0.329,P=0.742,P〉0.05);微脉冲组及传统组组内比较术后1-2月、3-5月CMT均显著下降(z=-2.086、一2.223,P=O.037、0.026)及(Z=-4.091、-3.621,P〈0.001、〈0.001);组问比较两组在黄斑水肿下降幅度上差异有统计学意义(z=-2.221、-2.756,P=0.026、0.006,P〈0.05)。结论577nm阈下微脉冲激光与传统激光治疗糖尿病黄斑水肿均有效,两者在短期内均能保持视力稳定,后者在短期内降低黄斑水肿高度方面好于前者。Objective To compare the efficacy of subthreshold micropulse 577nm laser and tradi- tional laser for diabetic macular edema (DME). Methods In a total of 28 cases (44 eyes) with DME who underwent routine ocular examination, FFA and OCT were enrolled in this study. The eyes were randomized into 2 groups for subthreshold micropulse 577nm laser (21 eyes) or tradition- al laser (23 eyes). The changes of best corrected visual acuity (BCVA) and central macular thick- ness (CMT) at 1-2 months, 3-5 months follow-up between 2 groups were compared. Results Be- fore the treatment, at 1-2 months and 3-5 months follow-up, the changes of BCVA between micro- pulse group and traditional group did not vary significantly (Z=-0.155, -0.624, and -0.258; P =0.877, 0.532, and 0.817). The differences in CMT between 2 groups did not vary significantly before the treatment (Z=-0.329, P =0.742). The changes of CMT at 1-2 months, 3-5 months follow-up was sig- nificantly in micropulse group (Z=-2.086 and-2.223, P =0.037 and 0.026) and traditional group (Z=-4.091 and -3.621, P 〈 0.001 and P 〈 0.001) respectively. But the reduction of CMT in tradition- al group was significantly higher than micropulse group (Z=-2.221 and -2.756; P =0.026 and 0.006). Conclusions Both subthreshold micropulse 577nm laser and traditional laser for DME are effective. Both of them can maintain visual acuity in the short time. The latter is more effective in reducing the CMT in the short time.
