摘  要:目的探讨巩膜外冷凝联合玻璃体腔全氟丙烷(C3F8)填充术治疗上方裂孔视网膜脱离的术后并发症,并对其特点加以分析。方法选取26例(26只眼)上方裂孔视网膜脱离患者行巩膜外冷凝联合玻璃体腔注射C3F8气体治疗。术中巩膜外冷凝裂孔位置,颞上或鼻上睫状体扁平部进针,注入浓度50%-100%长效气体C,F。约0.3-0.8ml,指测眼压正常,光感明确。术后保持面向下体位1周以上。观察其并发症。结果术后1月内6例发生高眼压;4例发生玻璃体混浊,其中3例1月后视网膜再次脱离,3例中2例为视网膜前增殖;16例发生晶状体可逆性混浊。结论巩膜外冷凝联合玻璃体腔注气治疗单纯上方裂孔视网膜脱离操作简单,适用于局限性裂孑L视网膜脱离的病人,但应重视这种手术方式的局限性,严格把握其适应证,避免术后并发症。Objective To investigate the clinical results and surgical complications associated with cryotherapy combined with C3F8 temponade in the treatment in superior rhegmatogenous retinal detachment. Methods Twenty-six consecutive patients with superior rhegmatogenous retinal de- tachment were included. Cryotherapy and C3F8 temponade were performed in all patients. Face-down position for 1 week was required after the surgery. Result Elevated lOP were noted in 6 eyes, vitreous opacities in 4 eyes within the 1st month post-operatively. Reversible posterior sub- capsular opacity was found in 16 eyes. Retinal redetachment were found in 3 eyes due to reopening of retinal tear (1 eye) or new tear (2 eyes) within ! month postoperatively. Proliferative vitreoretinop- athy were developed in 2 eyes. Further vitreotomy were performed in these 5 eyes. Conclusions Cryotherapy combined with C3F8 intraocular temponade is an effective, easy and less invasive sur- gery in superior retinal detachment. The post-operative complications including increased lOP, vitre- ous opacity, retinal redetachment and oroliferative retinopathy should be considered.
