摘　　要：目的探讨胚胎发育不良性神经上皮肿瘤的MR特征。方法回顾性分析17例经病理证实的胚胎发育不良性神经上皮肿瘤的MR表现。结果 17例中,位于额叶6例,颞叶4例,小脑半球3例,小脑蚓部、胼胝体、脑干及第四脑室各1例。MR中,病变信号与脑灰质相比,肿瘤在T1WI上呈等信号或等低信号,其中1例周缘可见弧形高信号影,在T2WI及FLAIR中以高信号为主,部分瘤内可见小囊状或不规则更长的T1、T2信号区,1例可见液—液平,钙化在T1WI及T2WI中均为低信号影;注射Gd-DTPA增强扫描,3例呈小片状强化,4例病变内部见结节样强化,5例瘤周呈环状强化,线样强化3例,2例未强化,2例瘤周可见轻度水肿。结论胚胎发育不良性神经上皮肿瘤的MR表现具有一定特征性,结合临床,可提高对此肿瘤的术前正确诊断率,确诊依靠病理检查。Objective To explore MR diagnosis of dysembryoplastic neuroepithelial tumor and improve diagnostic ca- pacity. Methods A retrospective analysis was made on MR of 17 cases with pathologically approved dysembryoplastic neuro- epithelial tumor. Results Among 17 cases, 6 cases were located in the frontal, 4 cases in the temporal lobe, 3 cases in the cerebellar hemisphere, 1 case in theivermis cerebelli,1 case in the corpus callosum, and 1 case in the brainsteml and fourth ventricle simultaneously. Compared with the signal intensity of the gray matter, all cases were presented as hypointensity or isointensity on T1WI, and 1 case showed circular hyperintensity on circumferentia, all of them were presented as hyperintensity on T2WI and FLAIR, some of them showed sacculiform or irregular longer T1 , T2 signal area, 1 case appeared liquid-liquid level, and calcification was the low signal in both T1WI and T2WI; After Gd-DTPA injection, 3 cases showed small pieces of enhancement, 4 cases of nodular lesions enhancement, 5 cases of ring enhancement, 3 cases of line-like enhancement, and 2 cases of no enhancement, 2 cases of peritumoral edema can be seen. Conclusion MR has superiority in the diagnosis of dyse- mbryoplastic neuroepithelial tumor, the diagnostic accuracy of preoperative can be increased through combined clinics, and confirmed diagnosis relied on pathological examination.
