摘  要:目的探讨阿托伐他汀降脂治疗对脑微出血的影响。方法 48例大动脉动脉粥样硬化性脑梗死合并脑微出血患者根据其血脂水平等分为强化降脂组(20例)、标准降脂组(14例)和非降脂组(14例);入组者在脑血管病常规治疗的基础上,强化及标准降脂治疗组分别给予阿托伐他汀平均(18.6±9.9)mg/d或(7.7±3.0)mg/d 3个月。治疗前后分别行磁敏感加权成像(SWI)检查脑微出血灶数以及血清总胆固醇(TC)、低密度脂蛋白(LDL)水平检测。结果强化及标准降脂组治疗后血清TC、LDL水平较治疗前显著下降(均P<0.05),非降脂组治疗前后血脂水平无明显变化;各组间治疗前后脑微出血灶数目差异无统计学意义。结论阿托伐他汀短期降脂治疗对脑微出血无明显影响。Objective To explore the influence of Atorvastatin on intracranial micro-bleed in lipid-lower therapy.Methods Forty-eight patients with large artery atherosclerosis cerebral infarction and intracranial micro-bleed were divided into strengthening lipid-lowering group(20 cases),typical lipid-lowering group(14 cases) and non-lipid-lower group(14 cases) according to thire lipid level ect.On the basis of routinecerebrovascular disease therapy,the patients in strengthening and typical lipid-lowering groups received Atorvastatin(18.6±9.9)mg/d or(7.7±3.0)mg/d respectively for 3 months.Pre and post therapy,the levels of serum total cholesterol(TC)and low density lipoprotein(LDL)were detected,the quantity of intracranial micro-bleed focus was observed by susceptibility weighted imaging(SWI).Results Compared with pre therapy,the levels of serum TC and LDL in strengthening and typical lipid-lowering groups post therapy were significantly lower(all P〈0.05),but was no significant difference in non-lipid-lower group.The difference of the quantity of intracranial micro-bleed focus pre and post therapy in each group was no statistical significance.Conclusion There is no significant influence of Atorvastatin on intracranial micro-bleed in short term lipid-lower therapy.
