卡马西平联合恩必普对脑卒中后癫痫患者认知功能的影响

摘　　要：目的:观察卡马西平联合恩必普(dl-3-正丁基苯酞)对脑卒中后癫痫患者认知功能的影响。方法:将45例脑卒中后癫痫患者随机分为联合用药组(23例)和对照组(22例)。联合用药组给予恩必普软胶囊0.2g,tid+卡马西平片0.1g,tid联合治疗;对照组给予卡马西平片0.1g,tid治疗,治疗随访半年。治疗前、后通过瑞文标准推理测验(SPM)进行认知功能的疗效评价和治疗前、后P300检测数据分析,同时观察2组的疗效及不良反应。结果:联合用药组的疗效显著优于对照组,2组比较差异有统计学意义(P<0.05);2组治疗后SPM测验情况均较治疗前显著改善(P<0.05);联合用药组治疗后P300波潜伏期及波幅改善情况显著优于对照组,2组比较差异有统计学意义(P<0.05)。2组不良反应较轻微,均可耐受。结论:卡马西平联合恩必普能显著改善脑卒中后癫痫患者的认知功能,且安全性较好。OBJECTIVE： To observe the effects of dl-3-bnolyphtha （Enbipu） and carbamazepine on cognitive function in post-stroke epilepsy. METHODS： Patients with post-stroke epilepsy were randomly divided into drug combination group （23 cases） and control group （22 cases）. Drug combination groups was given Enbipu soft capsule 0.2 g, tid ＋ carbamazepine 0.1 g, tid, con- trol groups was given carbamazepine 0.1 g, tid. Follow-up for all patients was at last 6 months, and the therapeutic efficacy was measured with raven standard progressive matrices test and data analysis of event-related potential P300 was conducted before and after treatment. RESULTS： Therapeutic efficacy of drug combination group was significantly better than those of control group, there was statistical significance between 2 groups （P^0.05） ; there was significant difference between 2 groups by raven standard progressive matrices test （P〈0.05） ; there was statistical significance in P300 latencies and amplitudes in 2 groups after treatment （P〈0.05）.The ADR of 2 groups were not serious. CONCLUSION： Enbipu soft capsule can improve cognitive function in patients with post-stroke epilepsy with good safety.
