多层螺旋CT在进展期胃癌术前分型及T分期的应用价值

摘　　要：目的探讨多层螺旋CT(MSCT)在进展期胃癌术前分型和T分期的应用价值。方法经手术病理证实的42例进展期胃癌患者,术前1周内采用16排螺旋CT平扫和动脉期、静脉期双期增强扫描并进行多平面重组(MPR)图像后处理,由两位腹部放射学高年资医师共同完成术前CT分型和T分期,并将所得结果与术后病理对照评价其准确率。结果 MSCT双期增强检查结合MPR技术后对进展期胃癌的检出率为100%,胃癌的术前MSCT大体分型和T分期总体准确率分别为85.7%和76.2%,其中T3期和T4期的胃癌CT分型的准确率高于T1期和T2期(p<0.05)。结论 MSCT双期增强扫描结合MPR后处理技术对进展期胃癌术前分型和T分期的准确率都较高,应将其作为胃癌术前常规必查项目。Objective To investigate the value of Multi - slice spiral CT （MSCT） in preoperative typing and T staging of advanced gastric carcinoma. Methods 42 patients with gastric carcinoma proved by surgical pathology were examined with 16 - slice spiral CT including plain scan and dual - phase enhanced scans and multiplanar reformation （MPR） of imaging data, respectively, one week prior to operation. All patients were classified as types and T stages by two experienced radiologists in abdominal field, and compared with pathologic findings. Results The detection rate of gastric carcinoma by MSCT with MPR technique was 100% for advanced gastric carcinoma. The accuracy of preoperative typing and T staging of advanced gastric carcinoma detected by the MSCT was 85.7% and 76. 2%, respectively. The accuracy of MSCT stages T3 and T4 are both higher than that of stages T1 and T2 （p 〈 0. 05 ）. Condusion The accuracy of preoperative CT typing and T staging performed by MSCT with MPR technique for advanced gastric carcinoma is higher. MSCT should be chosen as a routine method of preoperative examination.
