可吸收止血海绵在乳腺癌改良根治术中应用的效果观察

摘　　要：目的 探讨可吸收止血海绵在乳腺癌改良根治术后减少引流液的临床效果.方法 将200例可手术乳腺癌患者随机分成两组,手术方式均为改良根治术,对照组术后常规采取加压包扎持续负压引流,实验组则联合使用可吸收止血海绵填塞于腋顶区、腋静脉周围.分别观察患者术后第1、2天的引流液总量、两组患者拔管时间及皮下积液的发生率.结果 实验组术后前两天的总引流量平均为（150.70 ±39.20）ml,明显少于对照组的（199.45±58.18）ml,差异有统计学意义（P〈0.05）;置管时间实验组为（4.38±0.71）d,对照组为（6.15±1.07）d（P〈0.05）;实验组2例术后发生皮下积液,对照组9例（P〈0.05）.结论 乳腺癌改良根治术后引流管持续负压引流联合应用可吸收止血海绵可减少术后引流量、缩短置管时间、减少皮下积液的发生率.Objective To investigate the clinical effect of absorbable hemostatic sponge used in modified radical mastectomy of breast cancer to reduce drainage. Methods Clinical data of 200 patients with breast cancer were randomly divided into two groups. All patient＇s surgical procedures were modified radical mastectomy, among which the control group introduced pressure dressing and persistent negative pressure drainage, and the experimental group additionally used absorbable hemostatic sponge to fill in the axillary area, around the axillary vein. The drainage time, the total drainage volume in 2 days and the inci- dence of subcutaneous effusion were observed. Results The total drainage volume after 2 days in the ex-perimental group [ （150.70 ± 39.20 ）ml ] was significantly less than the control group[ （199.45 ± 58.18 ） ml] （P 〈0.05 ）;The drainage time in the experimental group was 4.38 ±0.71 days, and in the control group was 6.15 ± 1.07 days （P 〈 0.05 ） ; The incidence of subcutaneous effusion in experimental group was significantly lower （2%）, compared with the control group （9%） （ P 〈 0.05 ）. Conclusion The use of absorbable hemostatic sponge combined with persistent negative pressure drainage can reduce the time and wvolume of drainage and the incidence of subcutaneous effusion,thus shortening the hospital stay.
