综合护理联合抗抑郁剂治疗脑卒中后抑郁患者的临床研究

摘　　要：目的探讨综合护理联合抗抑郁剂对脑卒中后抑郁患者康复的影响。方法将脑卒中后抑郁患者65例随机分为干预组33例和对照组32例。两组患者均接受常规护理和康复治疗,干预组加用心理治疗等综合护理和抗抑郁药物治疗。治疗前后采用Hamilton抑郁量表(HAMD)、改良爱丁堡-斯堪的那维亚评分量表(MESSS)及简易精神状态量表(MESS)和Barthel(BI)指数对患者的抑郁程度、神经功能缺损程度、认知功能及日常生活功能进行评估。结果经过4周治疗,干预组抑郁症状明显好转(P<0.05),而对照组抑郁症状较治疗前无明显变化。治疗后两组患者认知功能及日常生活功能均明显提高,但两组之间比较干预组明显优于对照组(P<0.05)。结论综合护理联合帕罗西汀能有效缓解脑卒中后抑郁患者的抑郁状态,更有利于促进患者整体功能的恢复。Objective To investigate the effects of antidepressants plus comprehensive nursing intervention on recovery of patients with post- stroke depression （PSD）. Methods A total of 65 patients with PSD were randomly divided into observation group and control group. Two groups of patients received medical treatment and rehabilitation training, and the observation group also received the comprehensive nursing therapy combined with antidepressants. Hamilton Depression Scale （HAMD）, Modified Edinburgh-Scandinavian Scoring Scale （MESSS）, Mini-mental State Examination （MESS）, and Barthel Index （BI） were used to assess depressive symptoms, neurological functional deficit, cognitive function and daily living function. Results After four weeks of treatment, HAMD score of observation group significantly decreased, but that of control group had no change. The scores of MESSS, MESS and BI in two groups had significant improvement after treatment, while the improvement of observation group was significantly better than that of control group. Conclusion Antidepressants plus comprehensive nursing intervention could significantly improve depressive symptoms and function of patients with PSD.
