初期开展手法小切口白内障手术100例分析

摘　　要：目的分析初期进行手法小切口白内障手术时围手术期需注意的几个问题,以期对初学者有所裨益。方法回顾性分析2008～2009年笔者初期进行手法小切口白内障手术时收治的年龄相关性白内障患者共100例(104眼),男56例(58眼),女44例(46眼);年龄58～84岁。晶体核硬度2～4级,以3级核居多。结果术后3个月复诊时,56例(58眼)矫正视力≥0.8,26例(27眼)矫正视力0.4～0.8,18例(19眼)矫正视力0.3～0.4。术后并发症主要有角膜水肿及一过性高眼压。全组病例中有6例术中发生后囊破裂。结论初学者只有在对病例的选择、手术相关解剖知识的掌握及手术操作细节方面给予足够的重视,才能取得较好的手术效果,为下一步顺利开展该手术奠定良好的基础。【Objectives】 To analyze some aspects on manual small incision cataract surgery during perioperative period in early stage,and in order to be helpful to a beginner.【Methods】 About 100 cases（104 eyes） with age-related cataract were selected from 2008 to 2009,the early stage that I underwent manual small incision cataract surgery,including 56 males（58 eyes） and 44 females（46 eyes）,with age from 58 to 84.The hardness of lens nucleus was from grade 2 to grade 4,grade 3 was primary.【Results】 On the 3rd month postoperatively,the corrected visual acuity was 0.3~0.4 in 19 eyes,0.4~0.8 in 27 eyes,over 0.8 in 58 eyes.The mainly postoperative complications were corneal edema and transient high intraocular pressure.The rupture of posterior capsules was happened in 6 eyes.【Conclusions】 For a beginner,the surgery indications,anatomize knowledge and the manoeuvres should be paid more attention.Only in this way is it possible to achieve better results and lay a good foundation in the future.
