大肠管状腺瘤癌变过程中MMP-7的表达及意义

摘　　要：目的检测MMP-7在大肠管状腺瘤和大肠管状腺瘤癌变组织中的表达,探讨其在癌变过程中的作用,为临床诊断、治疗及预后判断提供新的理论依据。方法采用免疫组化EliVision两步法研究40例大肠黏膜、82例散发性大肠管状腺瘤、76例大肠管状腺瘤癌变组织中MMP-7的表达。结果大肠黏膜、散发性大肠管状腺瘤伴上皮不同程度异型增生和大肠管状腺瘤癌变组织中MMP-7阳性率分别为2.5%、32.9%、86.8%。随着病变程度的加深表达逐渐增高,差异具有统计学意义(P<0.05)。大肠管状腺瘤伴上皮不同程度异型增生组中MMP-7的阳性率分别为15.4%(轻、中度)、63.3%(重度),阳性率逐渐增高(P<0.05)。MMP-7在腺瘤癌变中的表达与年龄、肿瘤的分化程度、浸润深度、淋巴结转移及Dukes分期有关(P<0.05),与性别无关(P>0.05)。结论大肠管状腺瘤癌变过程中MMP-7的阳性率逐渐升高,MMP-7可能参与大肠管状腺瘤癌变的发生。Purpose To investigate the expression of MMP-7 in sporadic colorectal tubular adenoma and colorectal tubular adenoma with cancerous changes and to explore their role in the carcinogenesis of colorectal adenoma, in order to provide a theoretic basis for clinical diagnosis, treatment and predicting diagnosis. Methods The expression of MMP-7 was studied in 40 cases of colorectal muco- sa, 82 cases of sporadic c01orectal tubular adenoma with different dysplasia, and 76 cases of coloreetal tubular adenoma with cancerous changes using the two-step methods of EliVision plus. Results The positive expression rate of MMP-7 was 2. 5% , 32.9% and 86. 8% respectively in colorectal mucosa, sporadic eolorectal tubular adenoma with different dysplasia and colorectal tubular adenoma with cancerous changes, which was all significantly higher in the cases of coloreetal tubular adenoma with different dysplasia or with cancerous changes than that in colorectal mucosa （ P 〈 0. 05 ）, and also higher significantly in colorectal tubular adenoma with cancer- ous changes than that in colorectal tubular adenoma with different dysplasia （P 〈 0. 05 ）. The expression rate of MMP-7 was 15.4% in- dysplasia gradeⅠ and 63.3% in grade Ⅱ, respectively, in the colorectal tubular adenoma. Correlation was found between the expres- sion of MMP-7 and the age, tumor differentiation, the depths of the tumor invasion, lymphatic metastasis and Ducks stages （ P 〈 0. 05 ） in colorectal tubular adenoma with cancerous changes, and irrelevant with sex （P 〉 0. 05 ）. Conclusions Expression of MMP-7 in- creases gradually in the carcinogenesis process of sporadic colorectal tubular adenoma, which suggests that MMP-7 may be involved in the malignant transformation.
