摘 要:目的观察血管内皮生长因子(VEGF)与环氧化酶-2(COX-2)在恶性血液病中的表达,了解两者对血液肿瘤血管新生的作用及其可能存在的相互关系以及在急性白血病(AL)的不同病期表达的差异,探讨两者与疾病转归的相关性。方法应用酶联免疫吸附试验(ELISA)检测155例恶性血液病患者、40例非恶性血液病患者及20例健康体检者(共设为对照组)的血清中VEGF与COX-2的含量,分别对90例不同病期AL患者进行血清VEGF与COX-2含量测定,并对疾病转归进行随访。结果非恶性血液病患者VEGF(91.45±40.16)及COX-2(37.15±31.14),正常健康者(84.15±15.45)(29.35±26.63),两者差异无统计学意义(P>0.05)。恶性血液病患者VEGF(226.17±126.81)和COX-2(57.89±41.11)的表达水平显著高于对照组,差异有非常显著性(P<0.001),两种因子之间在表达上存在相关性,相关系数为0.575。在AL的不同病期存在VEGF和COX-2表达不同,VEGF表达在AL初治组(208.45±101.16)高于复发难治组(197.68±89.56)及AL-CR组(143.29±154.37);COX-2的表达在AL复发难治组(87.86±44.73)高于初治组(55.10±47.32)和AL-CR组43.04±36.36),AL-CR组与对照组之间存在差异性(0.05>P>0.01)。AL患者VEGF与COX-2表达与外周血白细胞计数及骨髓中原始细胞比例有正相关性,相关系数分别为r≈0.511和r≈0.562(P<0.05)。结论血液肿瘤患者存在VEGF与COX-2的高表达,两者在表达上可能存在相关性。AL不同病期VEGF与COX-2表达有差异,血液肿瘤患者进行VEGF与COX-2的检测,可能为AL病期判断、预测复发、评估预后的研究提供又一依据。Objective To observe the expressions of vascular endothelial growth factor{VEGF) and cyclooxygenase-2 ( COX-2 ) in hematologic malignancies, and to understand the functions and the possible relationship of VEGF and COX-2 in- volved in tumor angiogenesis;to observe the differential expressions of VEGF and COX-2 in acute leukemia (AL) of different disease stages, and to investigate their relationship with prognosis. Methods The enzyme linked immunosorbent assay (ELISA) was employed to test the serum VEGF and COX-2 contents in155 patients with malignant blood disease,40 cases with non-malignant blood disease and 20 healthy subjects ( total set as a control group ). The serum VEGF and COX-2 contents in 90 patients with different stages of AL were measured. The prognosis of the disease was followed Up. Results The contents of VEGF and COX-2 in the patients without hematologic malignancies (91.45 240. 16 and 37. 15 231.14, respectively} were not significantly different ( P 〉 0. 05 } from those in the normal healthy subjects ( 84. 15 + 15.45 and 29. 35 2 26.63, respective- ly ). The expression levels of VEGF (226. 17 + 126. 81 } and COX-2 { 57. 89 2 41.11 ) in patients with malignant blood disease were significantly higher than those in the control group ( P 〈 0. 001 ). And the expression of these two factors showed a correla- tion of 0. 575. The differential expressions of VEGF and COX-2 were observed in the AL of different disease stages. The expres- sion level of VEGF in untreated group { 208.45 ~ 101.16 ) was higher than that in the relapsed group ( 197. 68 2 89.56 ) and AL-CR group ( 143.29 2 154. 37 ). The expression level of COX-2 AL in the relapsed group ( 87.86 2 44. 73 ) was higher than that in the untreated group ( 55. 10 2 47. 32 ) and AL-CR group ( 43.04 2 36. 36 ).
