摘　　要：目的探讨选择性肝动脉灌注化疗栓塞术(TACE)联合经皮无水酒精注射(PEI)治疗乏血供肝癌的临床应用价值。方法选择经肝穿刺活检、CT增强及DSA检查为乏血供的肝癌50例,其中25例(联合治疗组)行TACE联合PEI治疗,25例(对照组)仅行TACE治疗,比较两组的治疗有效率、AFP转阴率及1年、2年生存率。结果联合治疗组和对照组治疗有效率分别为88%和52%,AFP转阴率分别为84%和48%,1年生存率分别为92%和68%,2年生存率分别为68%和32%。两组治疗有效率、AFP及患者1年、2年生存率比较差异有统计学意义(P<0.05)。结论 TACE联合PEI治疗乏血供肝癌安全、经济,可以提高TACE的疗效,特别对于小于3 cm的肿瘤,可以提高远期生存率。Objective To explore the clinical efficacy of transarterial chemoembolization（TACE） combined with percutaneous ethanol injection（PEI） in treating hypovascular liver tumors.Methods All 50 patients with hypovascular liver tumors confirmed by liver biopsy,contrast enhancement CT scanning and DSA were divided into two groups： the combination treatment group（25 cases） was treated with TACE and PEI,while the control group（25 cases） only with TACE.The rates of effectiveness,the rates of negative AFP and the 1-year and 2-year survival rates of both groups were compared.Results The rates of effectiveness was 88% in the combined treatment group and 52% in the control group;The rates of negative AFP were 84% in the combined treatment group and 48% in the control group;The 1-year and 2-year survival rates were 92% and 68% in the combined treatment group,and 68% and 32% in the control group,respectively.The difference in all indexes was conspicuously significant between the two groups（P0.05）.Conclusion The combination treatment with TACE and PEI is safe,cheap and more effective for the hypovascular liver cancer especially when the diameter of tumor is smaller than 3cm,and can improve the long-term surviva1 rate of patients.
