晚期青光眼视神经功能的保护及效果研究

摘　　要：目的观察甲钴胺保护晚期青光眼视功能的疗效。方法将72例(98眼)青光眼患者随机分成单纯小梁切除手术(A组,40眼)和小梁切除术后联合甲钴胺治疗(B组,58眼),对照分析术后两组患者的眼压、视力、视野、治疗后满意度等各方面的指标。结果术后两组眼压的控制率分别为87.50%(35眼)和91.38%(53眼),差异无统计学意义(P>0.05);甲钴胺治疗后视力改善率,A组为52.50%(21眼),B组为84.48%(49眼),两组间差异有统计学意义(χ2=2.417,P=0.016)。治疗后视野改善,两组相比差异也有统计学意义(t=2.684,P=0.022)。治疗后两组患者的满意度差别具有统计学意义,B组优于A组(F=5.142,P<0.01),并发症两组之间差异无统计学意义。结论小梁切除术联合甲钴胺药物治疗对晚期青光眼术后视功能的改善明显优于单纯小梁切除手术者。Objective To explore the effect of trabeculectomy and methycobal in the late glaucoma.Methods The patients（78 cases,98 eyes） were randomly divided into two groups： simple trabeculectomy（group A,40 eyes） and trabeculectomy with methycobal（group B,58 eyes）.The intraocular pressure,vision,visual field and degree of satisfaction of patients after the operations were compared between the two groups.Results IOP controlling rate after operation were 85.13%（35 eyes） and 92.14%（53 eyes）,respectively,and it had no significance between two groups（P0.05）.Visual acuity improving rate were 52.50%（21 eyes） and 84.48%（49 eyes）,respectively,and it had significance between two groups（χ2=2.417,P=0.016）.group B was superior to B group in visual field improving standard（Mean defect,MD） were 75.17% and 92.12%,respectively,and it had significance between two groups（t=2.684,P=0.022）.Conclusion Trabeculectomy with methycobal is superior to simple trabeculectomy for the late glaucoma.
