颈淋巴结转移性鼻咽癌调强放疗头部与颈部摆位误差比较研究

摘　　要：目的探讨有颈淋巴结转移的鼻咽癌调强放疗中头部和颈部摆位误差是否存在差异。方法采集头颈肩热塑面罩固定的17例中晚期鼻咽癌(T2-4N1-3M0)患者的模拟机位置验证图像,与首次计划设计的CT扫描数字重建影像(DRR)图像进行骨性参考点的偏离值比较,并采用双独立样本t检验头部、颈部摆位误差。结果头部摆位误差分别为:X轴(左右)(1.08±0.62)mm,Y轴(前后)(1.18±0.92)mm,Z轴(头脚)(1.08±0.99)mm;颈部摆位误差分别为:X轴(2.40±1.86)mm,Y轴(2.07±1.42)mm,Z轴(1.99±1.56)mm,两者之间差异具有统计学意义。结论有颈淋巴结转移的鼻咽癌患者,头部和颈部摆位误差存在明显的差异,应引起临床足够的重视。Objective To investigate the positioning errors of head and neck in the process of intensity modulated radiotherapy（IMRT） of neck metastasis nasopharyngeal carcinoma.Methods Seventeen patients with middle advanced nasopharyngeal carcinoma（T2-4N1-3M0） were treated by IMRT and underwent the immobilization by head neck-shoulder thermoplastic mask.We compared the deviation value of bone reference of CT scanning digital reconstruction image respectively x,y,z shifts relative to their position in the simulated machine.Results The positioning error of the head was（1.08±0.62） mm,（1.18±0.92） mm,（1.08±0.99） mm in x,y,z,respectively.And that of the neck was（2.40±1.86） mm,（2.07±1.42） mm,（1.99±1.56） mm,respectively.The positioning errors between neck and head have respectively statistical difference.Conclusion In the process of IMRT of neck metastasis nasopharyngeal carcinoma,the positioning errors of neck and head have respectively statistical difference,which we should pay more attention to.
