电刀排龈法和压线排龈法的临床应用效果比较

摘　　要：目的探讨电刀排龈在前牙区、前磨牙区和磨牙区的应用效果。方法收集需冠桥修复患者47例,共90颗基牙,其中前牙、前磨牙、磨牙各30颗。根据牙位分为前牙组、前磨牙组、磨牙组,每组又分为2个亚组,分别随机选取15颗采用电刀排龈法排龈,另外15颗采用压线排龈法排龈,比较2种排龈方法的操作时间和效果。结果前牙组、前磨牙组、磨牙组应用电刀排龈法的排龈时间分别为(2.07±0.10)min、(2.05±0.09)min、(2.14±0.13)min,平均排龈时间为(2.08±0.11)min;应用压线排龈法的排龈时间分别为(8.79±0.25)min、(8.51±0.23)min、(8.62±0.20)min,平均排龈时间为(8.64±0.25)min;2种方法的平均排龈时间差异有统计学意义(t=91.5,P<0.05)。前牙组、前磨牙组、磨牙组电刀排龈法排龈效果满意率分别为100%、100%、93%,平均满意率为97%;压线排龈法排龈效果满意率分别为80%、93%、100%,平均满意率为91%;2种排龈方法的平均满意率差异无统计学意义(χ2=1.90,P>0.05)。结论电刀排龈可以取得安全、满意的排龈效果,在所有牙位区的排龈操作时间均较压线排龈法明显缩短,在前牙区使用优越性较明显。Objective To compare the effect of electrosurgical retraction in the anterior teeth,the bicuspid teeth and the posterior teeth areas,with cord gingival retraction method.Methods Forty-seven patients and a total of 90 teeth werecollected,with 30 anterior teeth,30 bicuspid teeth and 30 posterior teeth.They were divided into anterior group;bicuspid group and posterior group.Each group were divided into two subgroups,with 15 teeth in each subgroup,for electrosurgical retractionand gingival retraction respectively.The time and the effect gingival retraction were observed.Results The time of electrosurgical retraction in anterior group,bicuspid group,posterior group was（2.07 ±0.10） min,（2.05±0.09） min,（2.14±0.13） min;the time of cord retraction in anterior group,bicuspid group,posterior group（8.79±0.25） min,（8.51±0.23） min,（8.62±0.20） min.There were significant differences in the time of two kinds of gingival retraction methods,（t=91.5,P〈0.05）.Satisfaction rate of electrosurgical retraction in anterior teeth group,bicuspid group,posterior group were 100%,100%,93%;while satisfaction rates of cord retraction in anterior teeth group,bicuspid group,posterior group t were 80%,93%,100%.The average satisfaction rate of electrosurgical retraction in three teeth areas was 97%,and the average satisfaction rate of cord gingival retraction in three teeth areas was 91%.There was no significant difference in satisfaction rate of the two kinds of gingival retraction（χ^2=1.90,P〉0.05）.Conclusion By electrosurgical retraction,a safe and satisfactory gingival retraction effect could beobtained,with less time of retraction operation and the high satisfaction rate than traditional cord gingival retraction method in anterior teeth.
