摘　　要：目的:定量检测肝病患者外周血可溶性细胞间黏附分子-1(soluble intercellular adhesion molecule-1,sICAM-1)的表达水平,以分析血清sICAM-1浓度对肝细胞性肝癌(hepatocellular carcinoma,HCC)诊断及预后监测的临床价值。方法:收集74例肝病患者血清及健康对照血32份,以酶联免疫吸附法定量检测血清sICAM-1水平,分析其临床意义。结果:肝病发展过程中血清sICAM-1表达呈梯度增加,HCC患者血清sICAM-1表达显著高于肝硬化患者(P<0.05)和正常对照组(P<0.01);与有无门脉癌栓、肝外转移明显相关(P<0.05);与患者年龄、性别、HBsAg阳性与否、AFP浓度及肿瘤大小间差异无统计学意义(P>0.05)。经受试者工作曲线分析,血清sICAM-1面积为0.872,sICAM-1表达异常,有助于HCC的诊断。结论:sICAM-1过表达与HCC发生、发展相关,血清sICAM-1异常是HCC诊断和转移监测的良好标志物。Objective： To investigate quantitatively the levels of circulating soluble intercellular adhesion molecule-1（sICAM-1） expression in a range of patients with liver diseases for exploring its diagnosis and monitoring prognosis for hepatocellular carcinoma（HCC）.Methods： The levels of circulating sICAM-1 were quantitatively detected in 74 patients with liver diseases and 32 health controls by ELISA,and analyzed clinical values.Results： The levels of serum sICAM-1 expression were increasing during the development of liver diseases.The expression of circulating sICAM-1 in HCC patients was significantly more than patients with liver cirrhosis（P0.05） and normal controls（P0.01）.There were no significant correlation between sICAM-1 expression and age,sex,AFP,positive-HBsAg and tumor size（P0.05） except of cancer embolus in portal vessel and extra-hepatic metastasis（P0.05） in HCC patients.The area under receiver operating characteristic curve of sICAM-1 expression was 0.872 and indicated that the abnormality of serum sICAM-1 level could be useful to HCC diagnosis.Conclusion： sICAM-1 over-expression is associated with occurrence and development of HCC,and circulating sICAM-1 level is a useful molecular marker in HCC diagnosis and monitor prognosis.
