眶内注射糖皮质激素治疗中度活动期甲状腺相关眼病的临床研究

摘　　要：目的探讨眶内注射糖皮质激素治疗中度活动期甲状腺相关眼病(thyroid-associated ophthalmopathy,TAO)的临床效果。方法选择47例68眼中度活动期TAO患者,根据患者眼眶CT,眶内注射曲安奈得及地塞米松混合液,于治疗前和治疗结束后1、6个月进行随访,观察治疗的近期及中远期疗效。结果糖皮质激素眶内注射治疗结束后1月,治疗有效53眼,有效率77.94%。治疗结束后6月,有效57眼,有效率83.82%。患者通过治疗结膜水肿、充血、眼睑肿胀、流泪等症状明显改善,眼球突出、眼睑退缩和眼外肌运动受限均有较大程度的缓解,无严重副作用及并发症发生。结论眶内注射糖皮质激素治疗中度活动期TAO安全、有效。Objective To evaluate the therapeutic effects of intraorbital injection of glucocorticoid for the treatment of moderate active thyroid-associated ophthalmopathy（TAO）.Methods 68 eyes of 47 patients with moderate active TAO were treated.According to the orbit CT scans,triamcinolone acetonide and dexamethasone were intraorbital injected.Therapeutic effects of the patients were observed after treatment 1,6 months.Results After intraorbital injection treatment 1 month,53 eyes（77.94%） were effective;After treatment 6 months,57 eyes（83.82%） were effective.Through the treatment,the symptoms of conjunctiva,palpebral edema and tears were obviously alleviated,proptosis,upper eyelid retraction and motility disturbances were improved.There were no serious side-effects and complications in all cases.Conclusion It is a safe and effective method that intraorbital glucocorticoid injection treated the moderate active TAO.
