脑室-腹腔分流术治疗蛛网膜下腔出血后迟发性脑积水26例临床研究

摘　　要：目的探讨蛛网膜下腔出血后迟发性脑积水的有效治疗手段。方法对本院2005年1月～2011年12月收治的26例迟发性交通性脑积水患者的情况进行回顾性分析,所有患者均在全麻下行脑室-腹腔(V-P)分流术,随访期为1～5年,综合头颅CT、MRI、术后症状改善及并发症评价其预后。结果 26例患者中,25例症状得到改善,22例明显改善。本组3例患者出现并发症,术后少量硬膜下血肿1例,少量硬膜下积液1例,未作特殊处理,保守治疗后吸收,1例术后半年分流管堵塞,行分流管调整术后缓解。此外,1例患者术后症状无改善。结论脑室-腹腔分流术对蛛网膜下腔出血后迟发性脑积水疗效肯定,值得推广。Objective To invstingate surgical treatment for tardus hydrocephalus after subarachnoid hemorrhage. Methods There were 26 cases with tardus hydrocephalus underwent ventricleperitoneal shunt therapy in our hospital from March 2005 to December 2010. All of the patients in the general downside ventricle-celiac （V-P） bypass surgery, follow-up 1 to 5 years, comprehensive head CT scan and MRI, postoperative symptoms improved and complications and evaluated its outcome. Results In 26 cases, 25 cases got satisfactory curative effect, significantly improved in 22 cases, 3 cases with complica- tions, after a few a subdural hematoma in 1 case, and a few subdural effusion in 1 case, not special treatment, after conser- vative therapy absorption and 1 case of postoperative half year bypass tube clogged, did the shunt tube adjustment to alle- viate. In addition, I case had the symptoms without improving. Conclusion Ventrieloperitoneal shunt can get significantly curative effect on tardus hydrocephalus after subarachnoid hemorrhage.
