摘  要:目的探讨准分子激光上皮瓣下角膜磨镶术(laser-assisted subepithelial keratomileusis,LASEK)和机械法准分子激光上皮瓣下角膜磨镶术(Epipolis laser in situ keratomileusis,Epi-LASIK)术后不同时间角膜中央知觉和泪膜稳定性恢复的情况。方法将60例(120眼)近视患者随机分为两组各30例(60眼),分别行LASEK和Epi-LASIK,对两组患者术后角膜中央知觉和泪膜稳定性的恢复情况随访24周,并行统计学分析。结果 LASEK组术后2周、4周角膜知觉计测定的角膜中央知觉分别是(44.85±5.78)mm和(49.85±5.01)mm,Epi-LASIK组则为(48.36±6.22)mm和(52.66±5.86)mm,术后2周和4周时LASEK组和Epi-LASIK组角膜中央知觉的结果比较差异均有显著统计学意义(均为P<0.01);术后12周LASEK组和Epi-LASIK组分别为(55.55±4.02)mm和(56.31±4.60)mm,两组比较差异有统计学意义(P<0.05);而术后24周两组分别为(57.31±3.11)mm和(57.62±2.68)mm,差异无统计学意义(P>0.05)。泪膜稳定性表现为术后2周、4周时非侵入性泪膜破裂时间LASEK组分别为(6.88±2.32)s和(9.06±3.15)s,Epi-LASIK组为(7.46±1.56)s和(9.62±1.29)s,两组比较差异均有统计学意义(均为P<0.05);12周、24周LASEK组的值为(11.21±5.13)s和(12.11±1.65)s,Epi-LASIK组分别为(12.06±0.98)s和(12.15±2.10)s,两组比较差异均无统计学意义(均为P>0.05)。结论通过对术后早期泪膜稳定性、角膜中央知觉的恢复比较,Epi-LASIK组患者术后刺激症状相对较轻,能较快恢复视觉。Objective To evaluate the corneal central consciousness and tear film stability at different time after laser-assisted subepithelial keratomileusis (LASEK) and Epipolis laser in situ keratomileus (Epi-LASIK). Methods Sixty patients ( 120 eyes) with myopia patients were randomly divided into two groups,30 cases (60 eyes) in each group, received LASEK and Epi-LASIK, respectively. The recovery of corneal central consciousness and tear film stability were followed up for 24 weeks after opera- tion,and the statistical analysis was performed. Results The corneal central con- sciousness at postoperative 2 weeks and 4 weeks in LASEK group were (44.85 ± 5.78 ) mm and (49. 85 ± 5.01 ) mm, respectively, which in Epi-LASIK group were (48.35 ± 6. 22) mm and (52.55 ±5.85 ) ram, there were significant differences ( both P 〈 0.01 ). The corneal central consciousness at postoperative 12 weeks of two groups was (55.55 ±4.02)ram and (56.31 ±4. 50) mm,there was statistical difference(P 〈0.05), which at postoperative 24 weeks of two groups were ( 57.31 ± 3.11 ) iron and ( 57.62 ± 2.58 ) mm, there was no statistical difference (P 〉 0.05 ). The tear film stability showed that,the non-invasive break-up time at postoperative 2 weeks and 4 weeks in LASEK group were ( 6. 88 ± 2. 32 ) s and ( 9.06 ± 3. 15 ) s, which in Epi-LASIK group were (7.46 ± 1.55 ) s and ( 9.62 ± 1.29 ) s, there were statistical differences ( both P 〈 0.05 ) ; The non-invasive break-up time at postoperative 12 weeks and 24 weeks in LASEKgroup were ( 11.21 ± 5.13 ) s and ( 12.11 ± 1.65 ) s, which in Epi-LASIK group were ( 12.06 ± 0.98 ) s and ( 12.15± 2.10) s, there was no statistical difference (both P 〉 0.05). Coneluaion By the comparison of postoperative early corneal central consciousness and tear film stability, the irritative symptoms after Epi-LASIK are less serious, the recovery of corneal con- sciousness is faster.
