联合激光虹膜切除术术后超声乳化晶状体摘出术治疗急性闭角型青光眼持续高眼压的疗效观察

摘　　要：目的探讨联合激光虹膜切除术后超声乳化晶状体摘出术治疗急性闭角型青光眼持续性高眼压的临床效果。方法选取2009年6月至2011年6月我院诊治的急性闭角型青光眼持续高眼压,并行联合激光虹膜切除术后超声乳化晶状体摘出+人工晶状体植入术患者48例(50眼),患者先行联合激光虹膜切除术,7～30d行超声乳化晶状体摘出+人工晶状体植入术;测量虹膜切除术前后眼压、晶状体摘出术后1周内眼压、4～6周眼压、随访6个月时的眼压,手术前后前房深度、房角关闭范围,术中、术后并发症。结果晶状体摘出术后核硬度为Ⅱ和Ⅲ级眼的眼压均低于术前眼压和虹膜切除术后的眼压,差异均有统计学意义(均为P<0.05)。中央前房深度由术前的(1.23±0.40)mm增加到虹膜切除术后(2.28±0.33)mm及晶状体摘出术后(3.32±0.38)mm,两两比较差异有显著统计学意义(P<0.01)。结论联合激光虹膜切除术后超声乳化晶状体摘出术可有效治疗急性闭角型青光眼持续高眼压,解除患者瞳孔阻滞,降低眼压。Objective To explore the efficacy of phacoemulsification after com- bined iris lasers for acute angle-closure glaucoma with sustained ocular hypertension. Methods Forty-eight patients （50 eyes） with acute angle-closure glaucoma with sus- tained ocular hypertension in our hospital from June 2009 to June 2011 were chosen,all cases received combined iris lasers firstly, and then phacoemulsification and intraocular lens implantation were performed from 7 days to 30 days after iridectomy. The intraocu- lar pressure before and after iridectomy, at 1 week,4 -5 weeks,5 months after phacoe- mulsification were measured, the anterior chamber depth, angle-closure status and com- plicatious were observed. Results The intraocular pressure in eyes with lens nucleus hardness at grade Ⅱ and Ⅲ after phacoemulsification were all lower than those before and after iridectomy, there were statistical differences （all P 〈 0.05）. The central anteri- or chamber depth before and after iridectomy, after phacoemulsification were （ 1.23 ± 0.40 ） mm, （ 2.28 ± 0.33 ） mm and （ 3.32 ± 0.38 ） ram, there were statistical differences between each two time point （P 〈 0.01 ）. Conclusion Phacoemulsification after com- bined iris lasers has obviously efficacy for acute angle-closure glaucoma with sustained ocular hypertension, the surgeries can relief the pupillary block and decrease the in- traocular pressure.
