35例安氏Ⅱ类1分类错双期矫正的临床分析

摘　　要：目的:探讨双期矫正在治疗安氏Ⅱ类1分类错畸形中的临床效果。方法:对35例安氏Ⅱ类1分类错患者进行双期矫正(Twin-block功能性矫治器+固定矫正),应用SPSS11.0软件包对治疗前后的X线头影测量数据进行配对t检验。结果:双期治疗结束后,患者后牙咬合,前牙覆、覆盖及侧貌均有明显改善,下颌生长朝着有利的方向发生改变。∠ANB、U1/SN、OB、OJ、UL-E、ULP在治疗后减少,差异具有统计学意义(P<0.01)。∠SNB、Co-Go、Go-Gn、Co-Gn在治疗后增加,差异具有统计学意义(P<0.01),L1-MP、FH-MP均显著增加(P<0.05)。结论:Twin-block矫治器对于安氏Ⅱ类1分类错患者具有矫治作用,通过诱导下颌功能性前移,抑制上颌骨生长,促进下颌骨生长,协调上、下颌骨关系,改善面型。PURPOSE： To evaluate the clinical value of the two-phase treatment of Angle Class Ⅱ division 1 malocclusion. METHODS： Thirty five cases with Class Ⅱ division 1 malocclusion were treated with two-phase treatment （Twin-block functional appliance and fixed appliance）.Lateral cephalograms were taken before and after treatment for each patient. The results were analyzed with paired t test using SPSS11.0 software package. RESULTS： After treatment ,the bite relation of molars ,the overbite and overjet and soft tissue profile were improved. ANB angle,U1-SN,OB,OJ,UL-E, ULP were decreased respectively （P〈0.01）. SNB, Co-Go,Go-Gn,Co-Gn were increased respectively （P〈0.01）. L1-Mp and FH-MP were increased, too（P〈0.05）. CONCLUSIONS： Twin-block functional appliance possesses the orthopedic effects on Angle Class Ⅱ division 1 malocclusion.It limits the maxillary growth forward and promotes the mandibular growth by repositioning the mandible forward.These may contribute to a harmonious relationship of upper and lower jaws and a better profile.
