胰腺体尾Ca继发脾梗塞及脓肿的CT表现的研究

摘　　要：目的探讨胰腺体尾癌累及脾动、静脉导致的脾梗塞、脾脓肿等螺旋CT表现及临床意义。方法回顾性分析16例胰腺体尾癌继发脾梗塞及脓肿患者的螺旋CT表现。结果16例均累及脾动、静脉脉，表现为脾动脉、静脉被推压、包绕，管径变细或粗细不均、边缘毛糙、血管强化程度降低及胃脾间出现迂曲、增粗血管影等征象，8例直接浸润脾脏，表现为胰尾、脾脏脂肪间隙模糊、密度增高，14例出现脾梗塞，表现为脾脏外围楔形低密度无强化灶，4例出现脾脓肿，表现为类圆形或蜂窝状低密度无强化灶。结论胰腺体尾Ca累及脾脏的螺旋CT表现多种多样，加深对其的认识能提高胰腺体尾Ca诊断正确率、全面性，减少误诊，为临床采取适当的治疗提供依据。Objective To discuss the CT features and clinic significance of splenic infarction and abscesses caused by carcinoma of body and tail of pancreas involved the splenic artery and spleen vein. Methods Retrospective analysis was conducted to study the manifestations of spiral CT of 16 cases of carcinoma of body and tail of pancreas with splenic infarction and abscess. Results The splenic artery and vein were invaded in all 16 cases, it showed the splenic artery and vein were pulled or bag around, the caliber thinner or uneven thickness, the edge was coarse, the degree of strengthening of splenic vessel reduced and appeared circuitous vascular shadows between the stomach and spleen clear- ance etc, 8 cases directly infiltrated the spleen, showing the fat clearance between pancreatic tail and spleen fuzzy and higher density, and 14 cases appeared splenic infarction, it showed the wedge non enhancement at periphery of the spleen, 4 cases appeared splenic abscess, with the qusic-circular or faviform non enhancement. Conclusion The CT features with carcinoma of body and tail of pancreas involving the spleen were infinite various, it can increase diagnostic accuracy of （＇.arcinmna of body and tail of pancreas through a deeper understanding, reducing misdiagnosis, and provides the basis for clinical take proper treatment.
