中西医结合治疗口腔扁平苔藓64例临床观察

摘　　要：目的观察中西医结合治疗口腔扁平苔藓的临床疗效及安全性。方法将120例患者随机分为治疗组和对照组,对照组56例单纯应用西药,病损基底部注射强的松龙混悬液0.5 mL加入利多卡因1.5 mL,混合应用,每周2次,5～7次为1个疗程;治疗组64例在对照组治疗的基础上配合中医辨证论治,分阴虚内热、脾虚夹湿、气滞血瘀3型,并根据主症选择不同治法和方药。观察两组患者口腔黏膜短期疗效和长期疗效。结果两组患者治疗后短期疗效(6～12个月)结果比较,治疗组总有效率为95.3%,对照组为37.5%,两组比较差异有统计学意义(P<0.05)。长期疗效(2年以上)结果比较,治疗组总有效率为100%,对照组为35.7%,两组比较差异有统计学意义(P<0.05)。结论中西医结合治疗口腔扁平苔藓疗效显著,不良反应少。Objective To observe the clinical efficacy and the safety of the integrated Chinese and western medicine on oral lichen planus. Methods 120 cases were randomized into a treatment group and a control group. 56 cases of the control group were treated with simple western medicine and the injection of the mixture of prednisolone suspension 0.5 mL and lidocaine 1.5 mL at the basement of the lesion, twice a week. 5 to 7 treatments made 1 session. In the treatment group, on the basis of the treatment as the control group, the cases were differentiated into 3 patterns/syndromes in terms of traditional Chinese medicine, named internal heat due to yin deficiency, mixture of spleen deficiency and damp, and qi and blood stagnation. The different treatment principles and the formulas were provided according to the chief symptoms. The short - term efficacy and the long - term efficacy of the oral mucous membrane were observed for the patients in two groups. Results In the comparison of the shortterm efficacy（6 to 12 months）for the patients between two groups, the total effective rate was 95.3% in the treatment group and was 37.5% in the control group, indicating the statistical significant difference（ P 〈 0.05 ）. In the comparison of the long -term efficacy（ over 2 years） ,the total effective rate was 100% in the treatment groups and was 35.7% in the control group,indicating the statistical significant difference （P 〈 O. 05 ）. Conclusion The integrated Chinese and western medicine achieves the significant efficacy on oral lichen planus and has less side reactions.
