HER家族在胃癌中表达及过表达的临床病理意义

摘　　要：【目的】通过检测表皮生长因子受体(HER)家族蛋白在胃癌组织中的表达,探讨HER家族蛋白表达或过表达及联合表达或过表达与胃癌临床病理特征之间的关系。【方法】采用免疫组织化学法检测124例手术切除的胃癌组织中HER家族蛋白的表达,分析HER家族蛋白表达、过表达、联合表达、联合过表达及其与胃癌临床病理特征及预后之间的关系。【结果】124例胃癌中HER家族蛋白HER1、HER2、HER3、HER4的表达率分别为41.1%(51/124)、59.7%(74/124)、82.2%(102/124)、28.2%(35/124);其过表达率分别为6.5%(8/124)、26.6%(33/124)、22.6%(28/124)、12.1%(15/124);癌旁非肿瘤胃黏膜均未见HER家族蛋白的表达;HRE2过表达在肠型胃癌较弥漫型胃癌更常见(P=0.001);HER3过表达与胃癌更深的浸润深度、易于淋巴结转移、易于远处转移、TNM分期更晚及预后差有关(P均<0.05);HER1、HER2和HER3联合表达与胃癌远处转移有关(P=0.035)。【结论】HER家族中HER3过表达可以作为评估胃癌恶性生物学行为的指标。[ Objective ] To investigate the correlation of expression or overexpression and co-expression or co-overexpression of HER family proteins with the clinicopathological parameters of gastric cancer. [ Method ] Expression of HER1 - 4 proteins were determined by immunohistochemistry in 124 gastric cancer tissues,the correlation of expression, overexpression, co- expression and co-overexpression of HER family proteins with clinicopathological parameters and prognosis of the patients were also analyzed. [Results] The expression rates of HER family proteins in 124 gastric cancer were 41.1% （51/124）, 59.7% （74/ 124）, 82.2% （102/124）, and 28.2% （35/124）, respectively. The overexpression rates of HER 1 ~ 4 proteins in the patients with gastric cancer were 6.5% （8/124）, 26.6% （33/124）, 22.6% （28/124）, and 12.1% （15/124）,respectively. HER2 overexpression was associated with intestinal-type gastric cancer（P = 0.001 ） ; HER3 overexpression was associated with depth of invasion, lymph node metastasis, distant metastasis, TNM stage of gastric cancer and worse survival （all P 〈 0.05） ; the co- expression of HER1, HER2, and HER3 were associated with distant metastasis of gastric cancer （P = 0.035）.[Conclusion] Overexpression of HER3 may be used to assess the malignant biological behavior and prognosis of gastric cancer.
