超声乳化人工晶状体植入联合房角分离术治疗闭角型青光眼临床观察

摘　　要：目的观察白内障超声乳化人工晶状体植入联合房角分离术治疗原发性闭角型青光眼合并白内障的临床效果。方法2008—06—2010—06于沧州市中心医院住院手术治疗的原发性闭角型青光眼合并白内障患者33例（39眼），男15例（18眼），女18例（21眼），年龄59-83（75．63±3．05）岁。所有手术均由同一手术医师完成，行透明角膜切口，晶状体超声乳化吸出后房型人工晶状体囊袋内植入联合房角分离术。于术前术后观察视力、最佳矫正视力、眼压、前房角、周边前房深度变化，并分析术后并发症。随访6～12个月。结果术后随访期间所有术眼眼压较术前显著降低，周边前房深度明显增加，29只眼（74％）房角完全开放，36只眼（92．31％）最佳矫正视力较术前提高。结论采用自内障超声乳化人工晶状体植入联合房角分离术治疗原发性闭角型青光眼合并白内障，能有效减低眼压，提高视力，开放房角，是安全按有效的方法。Objective To observe the clinical effect of cataract phacoemulsification and intraocular lens implantation joint with primary angle separation operation for treating angle-closure glaucoma complicated with cataract. Methods From June 2008 to June 2010 in our hospital there were 33 patients （39 eyes） with surgical treatment for primary angle-closure glaucoma complicated with cataract, in which there were 15 Male cases （18 eyes） and 18 female cases （21 eyes）. The age was from 59 to 83 and the average age was （75.63±3.05） years old. All were performed by the same surgery physicians with a transparent corneal incision, and phacoemulsification chamber intraocular lens im- plantation in bursa bag corner separation operation was performed. In the preoperative and postoperative the depth of visual acuity, best corrected visual acuity, intraocular pressure （IOP）, anterior chamber angle, and the peripheral anterior chamber changed, and the postoperative complications were analyzed. Follow-up continued for 6 to 12 months. Results After operation and in the follow-up period all eyes＇intraocular pressure were significantly reduced, peripheral anterior chamber depth significantly increased, 29 eyes＇ （74%） corner were completely open and 36 eyes＇ （92. 31%） best corrected visual acuity were improved. Conclusions cataract phacoemulsification and intraocular lens implantation joint with primary angle separation operation for treating angle-closure glaucoma compiicated with cata- ract can effectively decrease intraocular pressure, improve eyesight and open the corner. It is a safe and effective way.
