晶状体脱位继发青光眼高眼压状态下的手术治疗效果

摘　　要：目的观察晶状体脱位继发青光眼高眼压状态下的手术治疗效果。方法对23例(23眼)经充分降压后眼压仍很难降至正常的晶状体脱位继发青光眼患者,在高眼压状态下行手术治疗,所有患者随访6～12个月,观察术后视力、眼压及并发症情况。结果所有患者手术顺利,21例患者术后最佳矫正视力较术前提高,2例手术后视力无明显变化,术后1周和3个月眼压分别为(14.91±4.79)mmHg(1kPa=7.5mmHg)、(14.87±2.93)mmHg,与术前眼压(40.96±12.23)mmHg比较,差异均有显著统计学意义(均为P<0.01)。术中术后未出现严重并发症。结论晶状体脱位继发青光眼高眼压状态下及时手术可以控制眼压,挽救和改善患者视功能。Objective To explore the surgical treatment effect of secondary glau- coma patients with lens luxation under ocular hypertension. Methods Twenty-three patients （23 eyes） of secondary glaucoma associated with lens luxation were performed surgery, whose intraocular pressure （IOP） was higher than normal even after maximal medical treatment. The follow-up time was from 6 months to 12 months,and the visual acuity,IOP and complications were observed after operation. Results The surgeries in all cases were all right. The corrected visual acuity improved in 21 eyes,unchanged in 2 eyes. The postoperative IOP at 1 week and 3 months were （ 14.91 ± 4.79） mmHg （ 1 kPa =7.5 mmHg） and （ 14.87 ±2.93） mmHg,which were significantly lower than preoperative IOP（40.95± 12.23 ） mmHg,there were significant differences （ both P 〈 0. 01 ）. No intraoperative and postoperative serious complication happened in all cases. Conclusion The timely and suitable surgery can control IOP and improve the visual function of secondary glaucoma patients with lens luxation under ocular hypertension.
