抗青光眼术后不同角膜切口白内障超声乳化术对角膜屈光状态的影响

摘　　要：目的探讨抗青光眼术后不同角膜切口白内障超声乳化术对角膜屈光状态的影响。方法抗青光眼术后白内障患者56例(86眼),根据超声乳化术中切口不同分为2组,分别采用颞侧(A组)、上方透明角膜隧道切口(B组),观察2组患者术前、术后7d和30d裸眼视力、角膜散光度及并发症情况,并观察术后14d角膜、房水混浊程度。结果 2组术前和术后30d裸眼视力差异均无统计学意义(均为P>0.05),而术后7dA组裸眼视力高于B组,差异有统计学意义(P<0.05)。A、B组术前角膜散光度分别为(0.76±0.32)D、(0.80±0.25)D,术后7d分别为(2.42±0.73)D、(1.86±0.54)D,术后30d分别为(2.45±0.32)D、(2.15±0.36)D;2组术前、术后7d、30d角膜散光度差异均无统计学意义(均为P>0.05)。2组术后14d角膜、房水混浊程度差异均无统计学意义(均为P>0.05)。2组术后主要并发症为角膜水肿、虹膜损伤、后囊膜破裂。结论抗青光眼术后不同角膜切口白内障超声乳化术对角膜屈光状态的影响无显著差异,但颞侧切口术后视力恢复更快。Objective To discuss the effects of phacoemulsification with differ- ent corenal incision on corneal refraction after anti-glaucoma surgery. Methods Fifty- six patients （86 eyes） with cataract after anti-glaucoma surgery were divided into two groups,the temporal incision was used in group A, and upper incision in group B, the naked visual acuity, intraocuiar pressure, corneal astigmatism and complication at pre- operation,postoperative 7 days and 30 days in two groups were observed,and the cor- neal and chamber liquid opacity at postoperative 14 days were also observed. Results There was no statistical difference in naked visual acuity at pre-operation and postoper- ative 30 days between two groups（ both P 〉 0.05 ） ,but which at postoperative 7 days in group A was higher than that in group B （P 〈 0.05 ）. The preoperative corneal astigma- tism in group A and B were （0.76 ± 0.32） D and （0.80 ± 0.25 ） D, which at postoperative 7 days were （2.42 ± 0.73 ） D and （ 1.86 ± 0.54） D, at postoperative 30 days were （2.45 ±0.32 ） D and （ 2. 15±0.36 ） D, there was no statistical difference between two groups （ both P 〉 0.05 ）. There was no statistical difference in corneal and chamber liquid opacity at postoperative 14 days between two groups （ both P 〉 0.05 ）. The main complications in two groups were corneal edema, iris damage and posterior capsule rupture. Conclusion The effect of phacoemuisification with different incision on corneal refraction after anti-glaucoma surgery has no obviously difference, but the postoperative visual acuity recovery with temporal incision is more rapid.
