喉癌组织EGFR、p27和皮层肌动蛋白表达及其与局部复发的关系

摘　　要：目的探讨喉鳞状细胞癌(LSCC)组织中表皮生长因子受体(EGFR)、p27和皮层肌动蛋白(cortactin)表达与术后肿瘤局部复发的关系。方法采用免疫组化法检测60例喉癌组织原发灶EGFR、p27和皮层肌动蛋白表达情况并进行免疫染色评分。对患者临床病理因素及EGFR、p27和皮层肌动蛋白表达与喉癌术后局部复发情况进行多因素Logistic回归分析。结果多因素分析显示,EGFR、p27和皮层肌动蛋白表达是喉癌术后局部复发的独立影响因素(P=0.020,0.010,0.008),EGFR和皮层肌动蛋白高表达是局部复发的危险因素(β=1.244,1.639),p27高表达是局部复发的保护因素(β=-1.390)。喉癌术后局部复发与年龄、性别、临床分期和病理分化无关。结论EGFR、p27和皮层肌动蛋白表达与喉癌术后局部复发相关。EGFR、p27和皮层肌动蛋白三个因子可能成为预测喉鳞状细胞癌术后复发的生物标记物。Objective To investigate the relationship of Epidermal growth factor receptor （EGFR）, p27 and cortactin expressions in cancer tissues and local recurrence of laryngeal carcinoma. Methods The expressions of EGFR, p27 and cortactin in laryngeal squamous cell carcinoma（LSCC） were detected using immunohistochemistry in 60 patients with LSCC. The immunostaining scores for EGFR, p27 and cortactin were assessed. Multivariate logistic regression model was used to analyze the influence of the clinicopathological parameters and expressions of EGFR, p27and cortactin on local recurrence of LSCC. Results The multivariate analysis showed that the expressions of EGFR, p27 and cortactin in LSCC were independent influential factors for local recurrence（P=0. 020,0. 010,0. 008）. Higher expressions of EGFR and cortactin （β=1. 244, 1. 639） and lower expression of p27 （β=-1. 390） were accompanied by higher risk for local recurrence. The local recurrence of LSCC was not significantly correlated with age, gender, tumor stage or grade of tumor differentiation. Conclusion Local recurrence is associated with the expressions of EGFR, p27 and cortactin in LSCC. EGFR, p27 and cortactin may be the possible indicators for local recurrence in LSCC.
